OFFICIAL NOKIA AND AT&T WIRELESS $30 MAIL-IN REBATE FORM Instructions:

1. Purchase a Nokia 3360, Nokia 5165 or Nokia 6360 Digital phone between 7/14/02 and 10/19/02 from

an authorized AT&T Wireless representative. Activation on a qualified AT&T Wireless Digital monthly calling plan required. Offer also available with activation on AT&T Free2Go Wireless purchase of a Nokia 5165 Digital phone. Credit approval, minimum of one-year contract and activation fee required. Phone must remain on service for at least 30 days following activation and must be on AT&T Wireless service at the time the offer form is processed in order to be eligible for the $30 mail-in rebate. See AT&T Wireless General Terms & Conditions and brochures for the plan and features you choose for full terms and conditions.

2. Complete this form by providing all requested information. Incomplete forms will not be honored and will not be returned. (Please use black or blue ink and print using all CAPITAL LETTERS.)

3. Make a photocopy of the barcode sticker (1 1/2" x 4" sticker with various barcodes) found on the end of the product box that your Nokia 3360, Nokia 5165 or Nokia 6360 Digital phone originally came in. To make a photocopy, stand the box on its end, barcode down, on a copier and press the "print" button. If you have misplaced your phone packaging, please remove your battery from your phone and make a photocopy of your barcode on the back of your phone.

4. Make a photocopy of your original receipt or invoice from your AT&T Wireless representative that clearly shows the date of purchase for a Nokia 3360, Nokia 5165 or Nokia 6360 phone between 7/14/02 and 10/19/02. Please note: Incomplete submissions will not be honored and will not be returned.

Purchaser of service and phone is responsible for submitting the offer form. IMPORTANT: If your address changes, you replace your phone or need to send it in for repair after you have mailed in your offer form, please call 1-888-600-4480. Remember to keep your original barcode sticker, product packaging and your original receipt or invoice for your records. Any or all of these may be required for return or warranty service. Other Rules and Requirements: Calling plans may not be available for purchase or use in all areas.

All offer requests must be postmarked by 11/19/02 and received by 12/3/02. One $30 mail-in rebate for each qualifying Nokia 3360, Nokia 5165 or Nokia 6360 phone from an authorized AT&T Wireless representative. $30 mail-in rebate not available on phones that are returned or on refurbished handsets. $30 mail-in rebate offer valid while phone supplies last. b) All requests become property of Nokia and will not be returned. Use of multiple addresses or multiple P.O. boxes to obtain additional rebates is considered fraud, will be deemed void and invalidate all submissions, and may result in prosecution. Nokia and AT&T Wireless will not assume any responsibility for lost, late, damaged, stolen, misdirected, illegible, incomplete or postage-due requests or requests that fail to be properly delivered to the address stated below for any reason. Such requests will not be honored. c) Offer not available with any other Nokia rebates, offers or promotions. d) Please allow approximately 10 weeks for shipment of $30 rebate following receipt of offer form. e) Void where prohibited. By participating, you release Nokia and AT&T Wireless and their respective officers, directors, employees and affiliates from any and all liability. f) Rebate checks not cashed within 90 days from date issued are invalid and cannot be reissued. g) Make sure your fill-in grid includes name, address, purchase date and ESN number(s).
