TERMS AND CONDITIONS:

1. Microtek is not responsible for lost, stolen, or misdirected mail
2 Limit one rebate per name, address and household.

3. To receive your $20 rebate check, you must mail the following
three items: a) completed rebate claim coupon including
scanner serial number and your signature*; b) original UPC
label with numbers cut from the ScanMaker V6UPL original
product carton. No reproductions will be accepted; c) a
photocopy of your store-identified sales receipt with the
purchase price circled and date of purchase clearly visible.
(Please keep the original sales receipt for your records.)

4. Incomplete coupons will not be accepted. Coupons must have
serial number and customer's signature.

5. Clearly hand print information on rebate coupon.

6. Photocopy of your original sales receipt must be dated no
earlier than July 1, 2002 and no later than August 31, 2002.

7. Envelope containing rebate submission must be postmarked no
later than September 10, 2002. Faxed copies or copies sent via
email or as email attachments will not be accepted.

Rebate applies to the ScanMaker V6UPL and is valid

from July 1, 2002 to August 31, 2002.

Offer not valid with any other rebate, special retail discount
pricing, or any other promotions. Offer not valid with
ScanMaker V6UPL Office Edition.To receive your $20 rebate
check, you must mail the following three items:

15. Microtek will not honor rebate claim coupons with invalid or
duplicate product serial numbers, or invalid UPC codes.
