The IVAC®P7000 Actilyse is a fully featured high end variable pressure syringe pump suitable for critical care applications.

The IVAC

P7000 Actilyse variable pressure pump is one of the IVAC®family of "P" Series syringe pumps. Many aspects of general operation, performance, accuracy and safety checking systems designed into the P7000 unit are common throughout the range.

The IVAC

P7000 Actilyse functions with a range of standard, single use, disposable Luer-lock syringes

of various types and accepts sizes from 5ml to 100ml.

With the Actilyse dosing schedule option enabled, the pump can offer all the features of the IVAC

P7000

Actilyse or the ability for a pre-programmed multiphase protocol for the dispensing of the Actilyse drug.

100mg of Actilyse is reconstituted in 100ml of water for injections giving a fixed concentration of 1mg/ml.

Set-up of the Actilyse protocol requires entry of patient weight in the range 40kg to 150kg. Upon confirmation of weight the protocol is calculated as follows.

 Stage 1. Always set at 15mg at the cap bolus rate (typically 1200ml/h).  Stage 2. For patients with weight of 65kg and above, 50mg is infused in 30mins. For patients below

65kg dose is 0.75mg per kg of patient weight in 30min (maximum 50mg).

 Stage 3. For patients with weight of 65kg and above, 35mg is infused in 60mins. For patients below

65kg dose is 0.5mg per kg of patient weight in 60min (maximum 35mg).

Example 1 - 83kg patient.

 Stage 1 - 15mg bolus. Rate = cap bolus rate (typically 1200ml/h)  Stage 2 - 50mg dose. Given concentration of 1mg/ml volume to dispense is 50.0ml in 30mins giving

rate of 50.0 x 2 = 100ml/hr.

 Stage 3 - 35mg dose. Given concentration of 1mg/ml volume to dispense is 35.0ml in 60mins giving

rate of 35.0 x 1 = 35.0ml/hr.

Example 2 - 62kg patient.

 Stage 1 - 15mg bolus. Rate = cap bolus rate (typically 1200ml/h)  Stage 2 - 62kg x 0.75mg/kg = 46.5mg dose. Given concentration of 1mg/ml volume to dispense is

46.5ml in 30mins giving rate of 46.5 x 2 = 93.0ml/hr.

 Stage 3 - 62kg x 0.5mg/kg = 31.0mg dose. Given concentration of 1mg/ml volume to dispense is

31.0ml in 60mins giving rate of 31.0 x 1 = 31.0ml/hr.

Controls and Indicators

STOP Button - Press to stop the infusion. The Amber light will flash to indicate an alarm.

START Button - Press to start the infusion.

AC POWER - When illuminated the unit is connected to AC power and the internal battery is being charged.

Use “Double Chevron” for fast increase / decrease and the “Single Chevron” for slower increase / decrease of values shown on the display in set-up and configuration. Are also referred to as the rate keys.

MAIN DISPLAY - For screen information see Front Panel and Main Display.

Use “Blank Soft Keys” in conjunction with the prompts shown on the display.

OPTIONS button - Press the OPTIONS button to access drug name and other optional features.

Press PURGE/BOLUS to purge the extension line during set up while the pump is stopped or to bolus at an accelerated rate while an infusion is running.

BATTERY - When illuminated the pump is running on the internal battery. When flashing the battery power is low, less than 30 minutes of use remains.

ON/OFF - Press once to switch the pump ON. Press and hold down for 3 seconds to switch the pump OFF.

Operating Precautions

This IVAC®pump has been calibrated for use with single use disposable syringes. To ensure correct and accurate operation, only use LuerLock versions of the syringe make and size specified on the pump or described in the directions for use. Use of non-specified syringes or administration sets may impair the operation of the pump and the accuracy of the infusion.

Uncontrolled flow or syphoning may result if the syringe is located on the pump without its finger grips and plunger correctly located in the slots provided, or if it is removed from the pump before the extension line is properly isolated from the patient. Isolation may include closing a tap in the patient line or activating a flow stop clamp.

When combining several apparatus and/or instruments with administration sets and other tubing, for example via a 3 way tap, the performance of the pump may be impacted and should be monitored closely.

Do not mount the pump in a vertical position with the AC power inlet or the syringe pointing upwards. This could affect electrical safety in the event of a fluid spill over the unit or lead to an infusion of air which may be in the syringe. To protect against the introduction of air the user should regularly monitor the progress of the infusion, syringe, extension line and patient connections and follow the priming procedure specified in the purge procedure in Getting Started.

This is a positive pressure device designed to achieve very accurate fluid administration by automatically compensating for resistance encountered in the infusion system.

The pumping pressure alarm system is not designed to provide protection against, or detection of, infiltration conditions which can occur at low pressures.

Several alarm conditions detected by this pump will stop the infusion and generate audible alarms. Users must perform regular checks to ensure that the infusion is progressing correctly and no alarms are operating.

This instrument is protected against the effects of external interference, including high energy radio frequency emissions, magnetic fields and electrostatic discharge (for example, as generated by electrosurgical and cauterising equipment, large motors, portable radios, cellular telephones etc.) and is designed to fail safe if unreasonable levels of interference are encountered.

In some circumstances the unit may be affected by an electrostatic discharge of above 8kV. At test levels lower than these values the unit will operate normally. In rare circumstances the unit may be affected by radio frequency radiation above a level of 3V/m. If the unit is affected by this external interference the unit will fail safe or reset, (a call back alarm will occur after 2 minutes). Should false alarm conditions be encountered either, remove the source of the interference, or regulate the infusion by another appropriate means.

This unit emits a certain level of electromagnetic radiation which is within the levels specified by IEC60601-2-24 and IEC60601-1-2. If however the unit interacts with other equipment measures should be taken to minimise the effects, for instance by repositioning or relocation.

If this instrument is dropped, subjected to excessive moisture, humidity or high temperature, or otherwise suspected to have been damaged, remove it from service for inspection by a qualified service engineer.

An explosion hazard exists if the instrument is used in the presence of flammable anaesthetics. Exercise care to locate the unit away from any such hazardous sources. An electrical shock hazard exists if the units casing is opened or removed. Refer all servicing to qualified service personnel.

PROTECTIVE PACKAGING

Connect the unit to the AC power supply for 24 hours to ensure that the internal battery is fully charged. Should the pump fail to perform correctly, replace it in its original protective packaging and contact a qualified service engineer for investigation.

Pole Clamp

The pole clamp is supplied fitted to the rear of the unit and will provide secure fixing to standard I.V. poles of a diameter of up to 40mm. The pole clamp can also be fitted in a choice of 4 fixing positions allowing the unit to be mounted to vertical and horizontal poles, equipment rails and hospital furniture in a variety of convenient operating orientations. The pole clamp may be adjusted for use with horizontal fittings by using the existing fixings screws with the alternative fixing holes in the pole clamp. The multi position pole clamp hinge will support the unit at a range of angles on an I. V. pole. To adjust the angle the unit needs to be secured to a pole; using a hand at either end of the instrument, change the screen viewing and syringe access angle of the instrument.

Important: Do not mount the unit with the AC power inlet or the syringe pointing upwards. This could affect the electrical safety in the event of a fluid spill or lead to the infusion of air which may be in the syringe.

Replacing the AC Fuses

If the pump continually illuminates the battery symbol and the AC power indicator light does not illuminate when the pump is connected to the AC power supply and switched ON, suspect that either, the power supply fuse in the AC power plug, or, the internal AC fuses have blown. First check the power supply fuse in the AC mains plug. If the AC power indicator light does not illuminate remove the pump from service. It is recommended that only a qualified service engineer replaces the AC fuses. For further information regarding the replacement of the internal AC fuses refer to the technical service manual.

Functional Earth

It should be noted that the PSU fitted to this pump is functionally earthed. The functional earth feature has been designed to prevent electrical interference to other medical or electrical equipment. The AC power supply applied to this pump must have an earth conductor.

Important:

This provides a functional earth, not a protective earth.

Loading a Syringe

Getting SStarted

Place the pump on a stable horizontal surface or secure using the pole clamp fitted. Prepare, load and prime the single use disposable syringe and extension line using standard aseptic techniques.

1. Squeeze the finger grips on the plunger holder and slide the mechanism to the left. Lift the syringe clamp and rotate to the left.

2. Insert the syringe into the slots on the plunger holder.

3. Squeeze the finger grips on the plunger holder and slide the mechanism to the right until the syringe finger flanges locates in the V slot.

4. Release the finger grips. Apply gentle pressure on the plunger holder to ensure that the drive is engaged. Rotate the syringe clamp forward until it locks onto the syringe barrel.

5. Check that the syringe plunger and finger flanges are correctly located in their slots.

Important: Advance the syringe until the finger flanges touch the front of the V slot closest to the syringe clamp. This is important to prevent delay at the start of the infusion.

Important:

Only use a syringe of the type stated on the pump labelling.

Important: Only use a syringe of the type and size indicated on the pump or in this manual. Using an incorrect syringe could adversely affect the accuracy of the infusion and the performance of the pump. When initially loading the syringe, allow for the volume of fluid contained in the extension line and retained in the syringe at the end of infusion as this “deadspace” will not be infused.

Starting the Pump in Actilyse Mode

1. Connect the pump to an AC power supply using the AC power cable. Press the ON/OFF button.

2. NEW PATIENT ? - Answering NO will retain all previous patient history. YES will automatically reset the patient history to zero. Press the CONFIRM softkey to confirm the selection.

3. USE ACTILYSE ? - Answer YES to put the unit into the Actilyse set up screen.

4. WEIGHT - Enter patient weight using the chevron keys and press OK.

5. PROTOCOL DISPLAY - Check protocol displayed and press CONFIRM to continue or BACK to change patient weight. On confirmation of protocol check that time and date displayed are correct.

6. CONFIRM SYRINGE - Check that the syringe type and size matches the display. If required, the type of syringe can be changed by pressing the TYPE button. Press CONFIRM when the correct type and size are shown.

The display will show the current Actilyse stage and ACTILYSE flashed with ON HOLD.

7. PURGE (if required) - Press the PURGE/BOLUS button then the two PURGE buttons together until fluid flows and the syringe extension line priming is complete. The volume used during priming will be displayed on the display.

8. CONNECT PATIENT - Connect the extension line to the patient access site, and insert the pressure disc into the pressure transducer.

9. START - Press START to commence operation. INFUSING will be displayed and the progress of each of the three Actilyse stages will be shown on the right hand side of the display.

10. STOP - On completion of the Actilyse protocol the infusion will stop. The display will show the total volume infused. The instrument will return to normal mode operation.

Important: The unit will automatically operate from its internal battery if the pump is switched on without being connected to the AC power supply. Each time the unit is switched ON, check that the alarm beeps twice and that all the segments of the display, the green and amber lights are illuminated during the self test routine.

Purge

1. To use the purge option stop the pump and press the PURGE button.

2. The display will change and show two purge softkeys. Press the two PURGE softkeys simultaneously.

The purged volume is not added to the volume infused.

3. When the purging function has been completed press the QUIT softkey to exit the menu.

The PURGE button allows the delivery of a limited volume of fluid in order to purge the extension line prior to being connected to a patient or after changing a syringe. The purge function cannot be used if the rate lock is enabled.

Power ON/OFF

To power up the unit press the power on/off button once.

To power off the unit press and hold down the power ON/OFF button for 3 seconds. The display will count down and switch the unit OFF.

With the Actilyse option enabled the operation of the P7000 differs in the following ways:

 Rate Lock is permanently set and no option to change rate directly will be presented

regardless of the RATE LOCK setting in general options.

 Rate Titration is NEVER possible regardless of the RATE TITRATION setting in

general options.

 Bolus operation is NOT permitted during Actilyse operation.

3. CONFIRM SYRINGE - Check that the syringe type and size being used matches the display. If required, the type of syringe can be changed by pressing the TYPE button.

5. SET A RATE - Check the rate shown if old patient data has been retained and change the rate if necessary using the ADJUST buttons.

6. PURGE (if required) - Press the PURGE/BOLUS button then the two PURGE softkeys together until fluid flows and priming of the syringe extension line is complete. The audible alarm will operate during use of the PURGE softkeys and the volume used during priming will be shown on the display.

7. CONNECT PATIENT - Connect the extension line to the patient access device, and insert the pressure disc into the pressure transducer.

8. START - Press START to commence operation. INFUSING will be displayed.

The AMBER STOP light will be replaced by the flashing GREEN START light to indicate that the pump is operating.

9. STOP - Press STOP to halt operation. The AMBER light will replace the GREEN light.

Purge

Bolus Infusions

Important:

Alarms are not disabled during the operation of the bolus feature. The pressure alarms are temporarily increased to their maximum levels during the operation of the bolus feature. A bolus cannot be administered if the rate lock is enabled or if a multidose set-up is in use.

1. During infusion press the BOLUS button once to display the bolus screen.

2. Use the chevron keys to set the bolus dose required.

3. To deliver the bolus press the two BOLUS softkeys simultaneously. During the bolus the unit will display the volume being infused.

4. When the desired bolus has been delivered, release the two bolus softkeys. The bolus volume will be added to the total volume infused. Press QUIT to exit the bolus screen.

The bolus feature can be used during induction as well as during infusion.

If the volume of the bolus reaches the bolus volume limit the bolus will stop and the pump will automatically revert to infuse at the set rate. If the volume to be infused is reached during a bolus, the volume to be infused complete alarm will operate and the unit will revert to its previous state. Press MUTE to stop the alarm or CANCEL to acknowledge the alarm.

These three options enable the unit to be set-up for use with a specific drug and/or dosing protocol. Drugs are pre-configured (see configured options) to enable rapid selection of the drug name, dosing units and default rate. For increased security using a configured drug maximum and minimum safety limits are programmable for concentration and dose rates. The infusion does not need to be stopped in order to set up drugs and dosing.

When a protocol is used the drug name will be followed by a *. The set by ml/h / set by doserate option is now available.

7. Enter the dosing information prompted on the screen for the drug selected using the OK softkey.

8. Press the CONFIRM softkey to enter the drug name, dosing information and the protocol selected.

Important : The mass infused display is a sum of all drug masses infused which may be of different concentrations and even drug type. Therefore it should be noted that the relationship between currently displayed volume infused and mass infused may not directly relate to the current concentration.

This mode allows any dose rate set-up to be programmed with no reference to the preprogrammed drug protocols.

This option allows you to set a specific volume to infuse, and rate at the end of the volume to be infused, from stop, 1ml/h, 2ml/h and continuous infusion at the set rate.

Volume To Be Infused (VTBI)

1. Press the ? softkey to access the options menu.

2. Select the SET VTBI option using the chevron keys and press the OK softkey indicated on the screen.

3. Enter the volume to be infused using the chevron keys. When the volume to be infused has been entered, press the OK softkey.

4. Select the rate at the end of the VTBI using the rate selection chevrons. The default is STOP.

5. Press the OK softkey to exit the VTBI menu.

6. When the pump has delivered the set volume it will alarm. Press the CANCEL softkey to clear the alarm.

7. Press the CLEAR softkey to turn the VTBI function off, or set a new VTBI using the chevron keys.

Set VTBI Over Time

1. Press the ? button to access the options menu.

2. Select the SET VTBI OVER TIME option using the chevron keys and press the OK softkey indicated on the screen.

3. Adjust the volume to be infused using the chevron keys, when the volume to be infused has been entered press the OK softkey.

4. Enter the time over which the volume is to be infused. The infusion rate will automatically be calculated. Press the OK softkey to enter the value.

5. Select the rate at VTBI end and press the OK softkey. The default is STOP.

Set ‘VTBI Over Time’ must be activated in the Configured Options. This option allows a VTBI and delivery time to be specified. The rate necessary to deliver the required volume within the required time will be calculated and displayed. Infusion must be stopped to select this option.

Set by Doserate / Set by ml/h

To set rates accurately in doserate or flowrate increments it may be necessary to switch between the rate adjust options SET BY DOSERATE and SET BY ml/h. An arrow to the left of the rate display indicates the rate that will change when the chevron buttons are used to increase/decrease the infusion rate.

To set a doserate precisely the arrow must be pointing to the doserate (mg/kg/h); the flowrate will be calculated from the doserate.

To accurately set a flowrate the arrow must be pointing to flowrate (ml/h); the doserate will be calculated from the flowrate.

Selecting the Set By Doserate Option

1. Press the ? button to access the options menu.

2. Select the SET BY DOSERATE option using the chevron keys.

3. Press the OK softkey. This will select the set by doserate option and an arrow will automatically point to the doserate on the display. If necessary the doserate can be increased or decreased using the chevron keys.

Selecting the Set By ml/h Option

1. Press the ? button to access the options menu.

2. Select the SET BY ml/h option using the chevron keys.

3. Press the OK softkey. This will select the set by flowrate option and an arrow will automatically point to the flowrate on the display. If necessary the flowrate can be increased or decreased using the chevron keys.

Clear Volume

This option resets the volume level on the main display to zero.

1. Press the ? button to access the options menu.

2. Select the CLEAR VOLUME option using the chevron keys and press the OK softkey indicated on the screen.

3. The volume infused will be displayed.

4. Press the YES softkey to clear the volume infused displayed. Press the NO softkey to retain the volume.

Induction

The induction option is enabled within the general options. See General Options in this user manual.

1. Press the ? button to access the options menu.

2. Select the INDUCTION option using the chevron keys

3. Press the OK softkey indicated on the screen to confirm the selection.

4. Adjust the induction RATE using the chevron keys, then press the OK softkey.

5. Select the induction VOLUME using the chevron keys, then press the OK softkey. If an incorrect value has been entered, press the BACK softkey to return to the previous stage.

6. Enter the MAINTENANCE infusion rate using the chevron keys, then press the OK softkey.

7. Review the induction data on screen, then press CONFIRM. If necessary use the BACK softkey to return to the data. Once confirmed, the pump will return to the main display.

8. The induction can be cleared. Stop the infusion then press the ? button. Select CLEAR INDUCTION with the chevron keys and press the OK softkey to confirm.

8. Review the multidose data on screen, then press CONFIRM. If necessary use the BACK softkey to return to the data. Once confirmed, the pump will return to the main display. The review section of the display will show either multidose volume remaining or time between multidoses remaining.

24H Log

This option provides a graphical representation and an hour by hour record of the drug amount administered to the patient over the last 24 hour period.

1. Press the ? button to access the options menu.

2. Select the 24H LOG option using the chevron keys and press the OK softkey indicated on the screen.

3. Press the NEXT softkey indicated on the screen to access the hourly volume infused log.

4. Press the QUIT softkey to exit the log.

This option is enabled within the general options and can be modified to show the full contents of the log or the last patient records only.

The rate lock feature can be enabled in the general options of the pump.

When the infusion rate has been set and the infusion started, the rate lock prompt will appear in the main menu.

When the rate lock is enabled the rate cannot be altered if the pump is stopped. The infusion can be started and stopped in the normal way. The rate lock disables the bolus and purge operations.

1. Press the “?” button to access the options menu.

2. Select the UNLOCK RATE option using the chevron keys and press the OK softkey indicated on the screen.

Alarm Procedures

Alarms are indicated by a combination of an audible alarm, flashing amber STOP light and a descriptive message in the display.

1. A continuous audible alarm indicates that the infusion has stopped. First press MUTE to silence the alarm for a maximum of 2 minutes. Then check the display for a alarm message. Press CANCEL to cancel the alarm message.

2. When the cause of the alarm has been rectified, press the START button to resume the infusion.

PUMP DRIVE DISENGAGED - The drive system has been disengaged during the infusion. Check the finger grip and the position of the syringe.

EXCESSIVE DRIVE PRESSURE - Pumping pressure has reached the alarm limit. Following an occlusion the drive will backoff (if Fast-start and Back-off has been enabled - see general options) to reduce excess pressure in the line; alternatively squeeze finger grips on the plunger holder to release the drive mechanism and relieve any excessive pressure in the syringe and patient line. Identify and remove the cause of the blockage in the administration system before restarting the infusion.

SYRINGE CLAMP LOCATION - Incorrect size of syringe has been fitted, the syringe clamp has not been positioned correctly, or the syringe has been disturbed during operation. Check syringe location, position of syringe clamp, and syringe.

PLUNGER NOT DETECTED -The syringe plunger is not correctly fitted in the plunger holder. Check the syringe plunger location.

BATTERY CHARGE LOW WARNING - Battery charge low with up to 30 minutes operation remaining. Battery indicator will flash and after 30 minutes a continuous audible alarm will indicate that the battery is exhausted. Reconnect to AC power to continue operation and charge the internal battery.

Alarm Procedures

BATTERY EMPTY - Internal battery empty. To silence the alarm switch the pump OFF and reconnect the pump to AC power. Restart operation on AC power to charge the internal battery.

NEAR END OF SYRINGE WARNING (NEOI) - This is a timed event and can be set from 1 to 15 minutes. To set / change this value see the general options section in this user manual.

SYRINGE EMPTY - END OF INFUSION (EOI) -The pump has reached the end of the infusion. About 1% (can be set in configuration) of the syringe volume will remain in the syringe to minimise the risk of the infusion of air bubbles into the set.

AC POWER SUPPLY DISCONNECTED WARNING AC Power has been disconnected and the pump is operating on battery power. Reconnect AC power or press START to silence the alarm and continue battery operation. The display will flash AC POWER FAIL. The alarm will automatically cancel if the AC power is reconnected.

Important: If the AC power fail alarm is disabled in the general options, removing the AC power cable will not cause an alarm.

INTERNAL MALFUNCTION - The alarm system has detected an internal malfunction. Note the malfunction code. Remove the unit from service for examination by a qualified service engineer.

BATTERY MAY BE LOW/EMPTY - When the pump was last turned off the battery was low. If the pump has not been charged since then operate on AC power only.

21. Review the drug setup data, press the OK softkey to confirm the data displayed. This will return the pump to the drug setup menu. Use the BACK softkey at any time to return to the previous stage.

Fast Start and Backoff - Enables drive slack fast start and occlusion auto bolus reduction.

VTBI - Enables or disables the VTBI function.

Set VTBI over Time - Sets a specific volume to be infused over a fixed period of time.

Induction - Enables or disables the induction option.

Multidose - Enables or disables the multidose option.

Rate Lock - If enabled, the rate can be locked to prevent tampering of the set

infusion rate.

Quiet Mode - If enabled, the low priority alarms and button pushes are muted.

Pressure Alarm - Sets the pressure alarm value. Default is 300mmHg.

Max. Pressure Alarm - Sets the maximum pressure limit. Default is 750mmHg.

Auto Pressure Alarm - Enables or disables the automatic pressure alarm option.

AC Fail Alarm - Enables or disables the AC Fail Alarm.

Rate Titration - If enabled, the rate can be changed whilst the pump is infusing.

To titrate a rate while infusing, use the rate adjust keys to select the new rate and press the START button to confirm the new rate.

NEOI Warning - Sets the NEOI warning from 1 to 15mins.

EOI Point - Sets the end of infusion volume as a percentage of the syringe

volume from 0.5% to 5.0%.

KVO at EOI - If enabled the pump will switch to run at the KVO rate - 1.0ml/h

(or the current set rate if this is lower) at the EOI point. If disabled the pump will stop at the EOI point.

Beam Alarm - Sets up the default beam level between 0 and 7.

Patient Weight - Sets up the patient weight between 0.1kg and 150kg. Default is

70.0kg.

Purge Rate - Sets the purge rate between 100 and 500ml/h. The upper limit

prevents excessive loss of fluid whilst purging.

Bolus - Enables bolus delivery.

Default Bolus - Sets the default bolus rate between 10 and1200ml/h.

Cap Bolus Rate - Sets maximum permissible value for the bolus rate.

Cap Rate - Sets maximum permissible value for the infusion rate.

Display Event Log - If enabled the event log can be accessed via the options menu.

Log Last Patient Only - If enabled the event log shows only the last patient (since new

patient selected). If disabled the complete log can be viewed.

Nurse Call - Enables or disables the nurse call feature.

Nurse Call Inverted - When enabled the nurse call feature is inverted.

Comms Pump Address - Sets the Pump Communications Address.

Comms Monitor Only - When disabled, allows remote control of the unit from the comms

link. Monitor is always possible.

Comms Odd Parity - If enabled, communications parity is odd.

Comms ASCII - If enabled, communications protocol is in ASCII.

Preparazione all'impiego - Funzionamento Normale

Introduzione

IVAC®P7000 Alteplase è una pompa a siringa a pressione variabile, dalle elevate prestazioni, adatta per impiego aree di critical care. La pompa P7000 a pressione variabile della IVAC

è una delle pompe a siringa della serie „P". Molti degli aspetti generali di funzionamento, prestazione, precisione e dei sistemi di controllo della sicurezza concepiti per l'unità P7000 sono comuni a tutta la gamma. IVAC

P7000 funziona con una serie standard di siringhe Luer-Lock usa-e-getta di vario tipo ed è adatta per misure da 5ml a 100ml. Attivando l'opzione di programmazione del dosaggio di Alteplase, la pompa è in grado di offrire tutte le funzioni della IVAC

P7000 oppure di avvalersi di un protocollo preprogrammato, multifase, per la somministrazione della sostanza medicinale Alteplase. 100 mg di Alteplase vengono ricostituiti in 100 ml di acqua sterile per iniezioni, assegnando una concentrazione fissa di 1mg/ml. Per impostare il protocollo di Alteplase, si deve inserire il peso del paziente, compreso tra 40 kg e 150 kg. A conferma avvenuta del peso, il protocollo viene calcolato come segue:

 Punto 1. È sempre impostato su 15mg alla portata limite di bolus (tipicamente 1200ml/h).  Punto 2. Per pazienti di peso pari a 65kg e superiore, vengono infusi 50mg in 30min. Per pazienti di peso

inferiore a 65kg la dose è di 0,75mg per kg di peso del paziente in 30min (massimo 50mg).

 Punto 3. Per pazienti di peso pari a 65 kg e superiore, vengono infusi 35mg in 60min. Per pazienti di peso

inferiore a 65kg la dose è di 0.5mg per kg di peso del paziente in 60min (massimo 35mg).

Esempio 1 - Paziente di 83kg.

 Punto 1 - bolus da 15mg. Portata = portata limite di bolus (tipicamente 1200ml/h).  Punto 2 - dose da 50mg. Concentrazione data di 1mg/ml, volume da somministrare pari a 50,0ml in 30min.,

portata data di 50,0 x 2 = 100ml/h.

 Punto 3 - dose da 35mg. Concentrazione data di 1mg/ml, volume da somministrare pari a 35,0ml in 60min.,

portata data di 35,0 x 1 = 35,0ml/h.

Esempio 2 - Paziente di 62kg.

 Punto 1 - bolus da 15mg. Portata = portata limite di bolus (tipicamente 1200ml/h).  Punto 2 - 62kg x 0,75mg/kg = dose da 46,5mg. Concentrazione data di 1mg/ml, volume da somministrare

pari a 46,5ml in 30min., portata data di 46,5 x 2 = 93,0ml/h.

 Punto 3 - 62kg x 0,5mg/kg = dose da 31,0mg. Concentrazione data di 1mg/ml, volume da somministrare pari

a 31,0ml in 60min., portata data di 31,0 x 1 = 31,0ml/hr.

Controlli e indicatori

VISUALIZZATORE PRINCIPALE - Per informazioni sullo schermo cfr. Pannello frontale e visualizzatore principale.

Premere SPURGO/BOLO per spurgare il tubo di prolunga durante l'impostazione, mentre la pompa è ferma o per inviare un bolus ad una velocità accelerata durante la somministrazione dell'infusione.

Pulsante OPZIONI - Premere il pulsante OPZIONI per inserire il nome del medicinale e le altre caratteristiche opzionali.

ON/OFF - Premere una volta per accendere la pompa (ON). Premere e tenere abbassato il pulsante per 3 secondi per spegnerla (OFF).

TRASDUTTORE DI PRESSIONE - Rileva se è inserito un tset d’estensione munita di disco pressione. Il trasduttore misura le pressioni positive nel tubo per infusione.

Pulsante START - Premere per dare avvio all'infusione.

Pulsante STOP - Premere per interrompere l'infusione. La luce gialla lampeggia per segnalare un allarme.

ALIMENTAZIONE IN C.A. Quando è acceso, l'unità è collegata in c.a. e la batteria interna si sta caricando.

BATTERIA - Quando è acceso, la pompa sta funzionando mediante la batteria interna. Quando lampeggia, il livello della batteria è basso, restano ancora meno di 30 minuti di uso.

Utilizzare i „tasti programmabili" in associazione con i messaggi che appaiono sul visualizzatore.

Usare il tasto a „doppia freccia" per aumentare/diminuire velocemente e il tasto a „una freccia" per aumentare/diminuire lentamente i valori indicati sul visualizzatore durante le procedure di impostazione e configurazione. Questi tasti sono anche definiti „di velocità".

Questa pompa IVAC®è stata calibrata per essere impiegata con siringhe usa-e-getta. Per assicurare che il funzionamento avvenga in modo corretto ed accurato, usare solo la versione LuerLok dei tipi di siringa specificati sulla pompa o descritti nelle istruzioni per l'uso. L'uso di siringhe o di sistemi di somministrazione non conformi a specifica può danneggiare la funzionalità della pompa e ridurre la precisione dell'infusione.

Flusso incontrollato e sifonaggio possono aver luogo se la siringa è stata montata sulla pompa senza avere correttamente posizionato le impugnature sagomate e lo stantuffo nelle relative fessure oppure se è stata rimossa dalla pompa prima di avere opportunamente isolato il tubo di prolunga dal paziente. Per effettuare l'isolamento può essere necessario chiudere un rubinetto sul tubo collegato al paziente o mettere in funzione un morsetto arresta-flusso.

Quando si combinano diversi apparecchi e/o strumenti con sistemi di somministrazione e con altri tubi, p. es. su un rubinetto a 3 vie, il funzionamento della pompa può subire un ristagno e deve essere sottoposto a stretto monitoraggio.

Non montare la pompa in posizione verticale con l'ingresso per c.a. o la siringa rivolti verso l'alto, poiché ciò potrebbe influire negativamente sulla sicurezza elettrica, nel caso in cui un liquido dovesse fuoriuscire sull'unità, o portare a un'iniezione dell'aria eventualmente presente nella siringa. Per evitare l'ingresso dell'aria, l'utente deve monitorizzare regolarmente il procedere dell'infusione, la connessione di siringa, tubo di prolunga e paziente e seguire la procedura di riempimento specificata nella procedura di spurgo nel capitolo „Preparazione all'impiego".

Questo è un dispositivo a pressione positiva, concepito per ottenere una somministrazione di liquidi molto accurata, mediante compensazione automatica della resistenza incontrata nel sistema di infusione.

Il sistema di allarme della pressione di pompaggio non è concepito per rivelare o per offrire protezione in caso di condizioni di infiltrazione che possono verificarsi alle basse pressioni.

Parecchie delle condizioni di allarme rivelate da questa pompa interrompono l'infusione e generano un allarme sonoro. Gli utenti devono effettuare controlli regolari per accertarsi che l'infusione stia procedendo correttamente e non vi sia segnalazione di allarme.

Precauzioni d'uso

Questo strumento è protetto contro gli effetti di interferenza esterna, incluse le emissioni di radiofrequenza ad alta energia, da campi magnetici e da scarica elettrostatica (p. es. quelli generati da apparecchiature elettro-chirurgiche e di cauterizzazione, da grandi motori, da radio portatili, da telefoni cellulari, ecc.) ed è concepito in modo tale da bloccarsi nel caso in cui registri livelli di interferenza troppo elevati.

In talune circostanze l'unità può soggiacere ad una scarica elettrostatica superiore a 8 kV. A livelli di controllo inferiori a detti valori, l'apparecchio funzionerà normalmente. In rare circostanze l'unità può essere esposta a radiazione di radiofrequenza superiore al livello di 3 V/m. In caso di una tale interferenza esterna, l'unità si ferma o effettua un ripristino (l'allarme di richiamo scatta dopo due minuti). Se si dovessero verificare condizioni di falso allarme, procedere rimuovendo la causa dell'interferenza oppure regolando l'infusione in altro modo.

Questa unità emette un certo livello di radiazione elettromagnetica, compreso entro i livelli specificati dalle IEC 60601-2-24 e IEC60601-1-2. Se, ciononostante, l'unità dovesse interagire con altre apparecchiature, si devono adottare le misure necessarie a minimizzarne gli effetti, p. es. spostando la pompa o posizionandola in altro modo.

Se questo apparecchio dovesse cadere, essere esposto a eccessiva umidità o alta temperatura o se si avesse altrimenti il sospetto che sia stato danneggiato, toglierlo dal servizio e sottoporlo a ispezione presso un qualificato Centro di assistenza.

Vi è rischio di esplosione se lo strumento viene utilizzato in presenza di sostanze anestetiche infiammabili. Osservare la massima cautela e installare l'unità lontano da dette sorgenti di pericolo. Vi è rischio di scossa elettrica se l'involucro dell'unità è aperto o rimosso. Far effettuare tutte le operazioni di assistenza da personale di servizio qualificato.

Installazione

Controllare che la pompa sia completa, non danneggiata e che il voltaggio indicato sulla piastra di base sia compatibile con la Vs. alimentazione in c.a. La fornitura di questa pompa a siringa di ALARIS Medical Systems®comprende:

Collegare l'unità all'alimentazione in c.a. per 24 ore per garantire che la batteria interna sia completamente carica. Se la pompa non dovesse funzionare correttamente, riporla nell'imballaggio originale e contattare un Centro di assistenza qualificato per i controlli del caso.

Serraggio per braccio di sostegno

Il dispositivo di serraggio per braccio di sostegno è fornito montato sul retro dell'unità e garantisce un fissaggio sicuro ai bracci di sostegno standard dei dispositivi per IV di diametro fino a 40 mm. Il dispositivo di serraggio si presta anche per 4 diverse posizioni di fissaggio, permettendo di montare l'unità su bracci di sostegno sia verticali sia orizzontali, su barre di strumenti e su attrezzature ospedaliere, orientando l'apparecchio nelle svariate posizioni del caso. Il dispositivo di serraggio può essere adattato per il fissaggio su sistemi orizzontali, inserendo le viti di fissaggio esistenti nei fori alternativi previsti sul dispositivo di serraggio stesso. Detto dispositivo può anche essere fissato sulla base dell'unità, con possibilità di scelta tra 4 diverse posizioni. Un dispositivo opzionale di serraggio regolabile per braccio di sostegno è disponibile con il n° di ordine 6001FAOPT0H.

Importante : Non montare l'unità con l'ingresso per alimentazione in c.a. o con la siringa rivolti verso l'alto. Ciò potrebbe influire negativamente sulla sicurezza elettrica, nel caso in cui un liquido dovesse fuoriuscire, o portare a un'iniezione dell'aria eventualmente presente nella siringa.

Sostituzione dei fusibili per c.a.

Messa a terra funzionale

Si noti che l'alimentatore previsto su questa pompa ha una messa a terra funzionale. Questa caratteristica è stata concepita per evitare casi di interferenza con altre apparecchiature mediche o elettriche. L'alimentazione in corrente alternata a cui viene collegata questa pompa deve essere dotata di conduttore di terra.

Importante:

Ciò garantisce una messa a terra funzionale e non di protezione.

Se il simbolo della batteria presente sulla pompa rimane continuamente illuminato e la luce di segnalazione del collegamento in c.a. non si illumina quando la pompa è alimentata in c.a. e accesa, si suppone che il fusibile d'alimentazione, interno sia saltato. Si raccomanda di far sostituire il fusibile di rete solo da un qualificato Centro di assistenza. Per ulteriori informazioni in merito alla sostituzione dei fusibili interni per c.a. fare riferimento al manuale tecnico di servizio.

Caricare una siringa

Appoggiare la pompa su una superficie orizzontale stabile o assicurarla mediante il dispositivo di serraggio per braccio di sostegno. Preparare, caricare e riempire la siringa usa-e-getta e il tubo di prolunga secondo le tecniche asettiche standard.

1. Premere le impugnature sagomate sul porta-stantuffo e far scivolare il meccanismo verso sinistra. Sollevare la graffa della siringa e ruotare verso sinistra.

2. Inserire la siringa nelle fessure sul portastantuffo.

3. Premere le impugnature sagomate sul porta-stantuffo e far scivolare il meccanismo verso destra sino a che i bordi della siringa vengano a trovarsi nella fessura a V.

4. Rilasciare le impugnature sagomate. Applicare una delicata pressione sul portastantuffo per accertarsi che il comando sia innestato. Ruotare in avanti la graffa della siringa sino a che si blocchi sul cilindro della siringa.

5. Controllare che lo stantuffo e i bordi della siringa siano correttamente posizionati nelle rispettive fessure.

Importante: Far avanzare la siringa sino a che i bordi tocchino la parte frontale della fessura a V più vicina alla graffa della siringa. Ciò è importante per prevenire un ritardo all'avvio dell'infusione.

Usare esclusivamente una siringa del tipo indicato sulla targa della pompa.

Importante: Usare esclusivamente una siringa del tipo e della misura indicati sulla pompa o in questo manuale. L'impiego di una siringa non adatta può influire negativamente sulla precisione dell'infusione e sul funzionamento della pompa. Caricando inizialmente la siringa, tenere presente il volume del liquido contenuto nel tubo di prolunga e restante nella siringa alla fine dell'infusione, poiché questo „spazio morto" non viene infuso.

7. SPURGO (se necessario) - Premere il pulsante SPURGO/BOLO e poi i due pulsanti SPURGO contemporaneamente, sino a che il liquido fluisce e il tubo di prolunga della siringa è completamente pieno. L'allarme sonoro è in funzione durante l'uso dei pulsanti SPURGO e il volume impiegato per la procedura di riempimento viene visualizzato sullo schermo.

8. COLLEGARE PAZIENTE - Collegare il tubo di prolunga al punto di accesso sul paziente, e inserire il disco pressione nel trasduttore.

9. START - Premere START per iniziare l'operazione. Sul visualizzatore compare INFUSIONE e la successione di ciascuna delle tre fasi di Alteplase viene indicata sulla destra del visualizzatore.

10. STOP - Al completamento del protocollo Alteplase l’infusione si fermerà. Il display mostrerà il volume totale infuso. Lo strumento ritornerà al modo operativo normale.

Importante: L'unità funziona automaticamente per mezzo della batteria interna, se la pompa è accesa senza essere collegata all'alimentazione in c.a. Alimentazione ON/OFF

Per attivare l'unità, premere una volta il pulsante di alimentazione ON/OFF. Per disattivare l'unità premere e mantenere abbassato il pulsante ON/OFF per 3 secondi. Il visualizzatore esegue il count-down e spegne l'unità.

Spurgo

Il pulsante PURGE (spurgo) permette di somministrare una limitata quantità di liquido per spurgare il tubo di prolunga prima di collegarlo al paziente o dopo aver sostituito la siringa. La funzione di spurgo non può essere utilizzata se si è attivato il blocco portata.

1. Per usare la procedura di spurgo, fermare la pompa e premere il pulsante SPURGO.

2. Il visualizzatore cambia e indica due tasti di spurgo. Premere i due tasti SPURGO contemporaneamente.

Il volume del liquido di spurgo non viene aggiunto al volume infuso.

3. Quando la funzione di spurgo è stata portata a termine, premere il tasto USCIRE per uscire dal menu.

Differenze di Funzionamento di P7000 Alteplase

Quando l'opzione Alteplase è attivata, il funzionamento di P7000 differisce come segue:

 Il blocco portata è impostato in permanenza e non viene presentata nessuna

opzione atta a modificare direttamente la portata, indipendentemente da come è impostato RATE LOCK nelle opzioni generali.

 Non è mai possibile titolare la portata, indipendentemente da come è impostato

RATE TITRATION nelle opzioni generali.

3. CONFERM. SIRINGA - Controllare che tipo e misura della siringa che si vuole usare siano conformi a quanto visualizzato. Se necessario, il tipo della siringa può essere modificato mediante il pulsante TIPO.

4. Premere CONFERM. quando sono indicati tipo e misura corretti.

5. IMPOSTARE LA PORTATA - Controllare il valore della portata indicato, se è stata mantenuta l'anamnesi precedente del paziente, e modificarlo, se necessario, mediante i pulsanti REGOLARE.

6. SPURGO (se necessario) - Premere il pulsante PURGE/BOLO e poi contemporaneamente i due tasti di spurgo (PURGE) sino a che il liquido fluisce e lo spurgo del tubo di prolunga della siringa è completo. L'allarme sonoro è in funzione durante l'uso dei tasti PURGE e il volume impiegato per la procedura di riempimento viene visualizzato sullo schermo.

7. COLLEGARE PAZIENTE - Collegare il tubo di prolunga al dispositivo di accesso del paziente e inserire il disco pressione nel trasduttore.

8. START - Premere START per iniziare l'operazione. Appare INFUSIONE sul visualizzatore.

La luce GIALLA di STOP viene sostituita dalla luce lampeggiate VERDE di START, per indicare che la pompa è in funzione.

Spurgo

1. Per usare la procedura di spurgo, fermare la pompa e premere il pulsante SPURGO.

2. Il visualizzatore cambia e indica due tasti di spurgo. Premere i due tasti SPURGO contemporaneamente.

Il volume del liquido di spurgo non viene aggiunto al volume infuso.

3. Quando la funzione di spurgo è stata portata a termine, premere il tasto USCIRE per uscire dal menu.

Infusioni di bolo

Importante: Gli allarmi non sono disattivati durante l'operazione di bolo. Gli allarmi relativi alla pressione sono temporaneamente portati al loro livello massimo durante l'operazione di bolo. Il bolo non può essere somministrato se il blocco portata è attivato o se si sta usando un'impostazione multidose.

La funzione di bolo può essere utilizzata sia durante l'induzione, sia nel corso dell'infusione. Quando il volume del bolo raggiunge il proprio valore limite, il bolo si interrompe e la pompa

continua automaticamente a somministrare l'infusione secondo la portata impostata. Se si raggiunge il volume da infondere durante un bolo, entra in funzione l'allarme di „volume da infondere (VTBI) completo" e l'apparecchio ritorna alla condizione precedente. Premere SILENZIO per fermare l'allarme oppure ANNULLARE per riscontrarlo.

1. Durante l'infusione premere una volta il

pulsante BOLO per far comparire la schermata relativa.

2. Usare i tasti a freccia per impostare la dose

di bolo desiderata.

3. Per inviare il bolo, premere

Quando ci si avvale di un protocollo, il nome del medicinale è seguito da un * L'opzione „impostare secondo ml/h / Impostare secondo intensità di dose" è ora disponibile.

1. Premere il pulsante ? per accedere al menu opzioni.

2. Selezionare l'opzione FARMACI E DOSAGGIO mediante i tasti a freccia.

3. Premere il tasto OK indicato sullo schermo per confermare la selezione.

4. Selezionare il nome di un medicinale nella lista visualizzata mediante i tasti a freccia, premere il tasto SI indicato sul visualizzatore per confermare la selezione.

5. Premere il tasto SI indicato sul visualizzatore per selezionare DOSAGGIO.

6. Premere il tasto SI indicato sul visualizzatore per selezionare PROTOCOLLO. In tal modo si seleziona il protocollo predefinito per il nome del medicinale selezionato.

7. Inserire l'informazione sul dosaggio del medicinale selezionato, comparsa come messaggio sullo schermo, premendo il tasto

8. Premere il tasto CONFERM. per inserire il nome del medicinale, l'informazione sul dosaggio e il protocollo selezionati.

Queste opzioni permettono di impostare l'unità per l'impiego di uno specifico protocollo per medicinale e/o dosaggio. I medicinali sono pre-configurati (cfr. Opzioni di configurazione) per rendere possibile una rapida selezione del nome del medicinale, delle unità di dosaggio e della portata predefinita. Per una maggiore sicurezza, quando si utilizza un medicinale della configurazione è possibile programmare i limiti massimo e minimo della concentrazione e dell'intensità di dose.

Attenzione :

Il visualizzatore massa infusa indica la somma di tutte le masse di medicinali infusi, che possono essere di diversa concentrazione e persino di diverso tipo. Si deve pertanto considerare che il rapporto tra il volume infuso, attualmente visualizzato, e la massa infusa può non riferirsi direttamente all'attuale concentrazione.

3. Regolare il volume da infondere mediante i tasti a freccia. Quando detto volume è stato inserito, premere il tasto OK.

4. Inserire il tempo nel corso del quale il volume deve essere infuso. La portata dell'infusione viene calcolata automaticamente. Premere il tasto OK per inviare il valore.

5. Selezionare la portata alla fine di VDI e premere il tasto OK. Il valore predefinito è STOP.

Questa opzione permette di impostare un volume specifico che si vuole infondere e la portata alla fine di detto volume, a partire da stop, 1 ml/h, 2 ml/h e infusione continua alla portata impostata.

Impostare VTBI nel tempo

Si deve attivare „Impostare VTBI nel tempo" nelle opzioni della configurazione. Questa opzione permette di specificare il VTBI e il tempo relativo di somministrazione. Viene calcolata e visualizzata la portata necessaria a somministrare il volume desiderato nel tempo desiderato. Per poter selezionare questa opzione, si deve interrompere l'infusione.

Volume da infondere (VDI)

1. Premere il tasto ? per accedere al menu opzioni.

2. Selezionare l'opzione SCELTA VDI mediante i tasti a freccia e premere il tasto OK indicato sullo schermo.

3. Inserire il volume da infondere mediante i tasti a freccia. Quando lo si è inserito, premere il tasto OK.

4. Selezionare la portata alla fine di VDI, mediante i tasti di velocità. Il valore predefinito è STOP.

5. Premere il tasto OK per uscire dal menu VDI.

6. Quando la pompa ha infuso il volume programmato scatterà l'allarme. Premere il tasto CANCEL per disattivare l'allarme.

7. Premere il tasto CANCELLA per azzerare il VTBI, o programmare un nuovo VTBI utilizzando I tasti con le frecce.

Per impostare accuratamente i valori di incremento avvalendosi dell'intensità di dose oppure della portata, può essere necessario passare dall'opzione SCELTA FLUSO DI DOSE a quella SCELTA ml/h e viceversa. Una freccia posta alla sinistra del visualizzatore dati indica il valore modificato quando si usano i tasti a freccia per aumentare/diminuire il valore di infusione.

Per impostare con precisione l'intensità di dose, la freccia deve puntare sul valore relativo (mg/kg/h); in tal caso la portata sarà calcolata in base all'intensità.

Per impostare accuratamente la portata, la freccia deve puntare sul valore relativo (ml/h); l'intensità di dose sarà calcolata in base alla portata.

Cancellare il volume

1. Premere il pulsante ? per accedere al menu opzioni.

2. Selezionare l'opzione CANCELL. VOLUME mediante i tasti a freccia e premere il tasto OK indicato sullo schermo.

3. Il volume infuso compare sul visualizzatore.

4. Premere il tasto SI per cancellare il volume infuso visualizzato. Premere il tasto NO per conservare il volume.

Induzione

L'opzione di induzione si attiva nell'ambito delle opzioni generali. Cfr. Opzioni generali in questo manuale.

1. Premere il pulsante ? per accedere al menu opzioni.

2. Selezionare l'opzione INDUZIONE mediante i tasti a freccia.

3. Premere il tasto OK indicato sullo schermo per confermare la selezione.

4. Regolare la portata (VELOCITA) di induzione mediante i tasti a freccia, poi premere il tasto OK.

5. Selezionare Il VOLUME di induzione mediante i tasti a freccia, poi premere il tasto OK. Se si è inserito un valore non corretto, premere il tasto INDIETRO per ritornare al passaggio precedente.

6. Inserire la portata d'infusione per il MANTENIMENTO mediante i tasti a freccia, poi premere il tasto OK.

7. Rivedere i dati di induzione sullo schermo, premere poi CONFERM. Se necessario usare il tasto INDIETRO per ritornare ai dati. Una volta confermato, la pompa ritorna al visualizzatore principale.

8. Si può azzerare l'induzione. Interrompere l'infusione, poi premere il pulsante ?. Selezionare ELIMINA INDUZIONE con i tasti a freccia e premere il tasto OK per confermare.

Una volta confermato, la pompa ritorna al visualizzatore principale. Il settore revisione del visualizzatore indica il volume multidosi restante oppure il tempo restante tra le multidosi stesse.

Multidose

La funzione multidose si attiva nell'ambito delle opzioni generali. Cfr. Opzioni generali in questo manuale.

La funzione di blocco portata può essere attivata nelle opzioni generali della pompa. Quando si è impostata la portata dell'infusione e si è iniziata l'infusione stessa, il messaggio VELOCITA BLOCCATA compare nel menu principale. Quando si attiva questa funzione, la portata non può essere modificata se la pompa viene fermata, l'infusione può essere iniziata e interrotta in modo normale. Il blocco portata disattiva le operazioni di bolo e spurgo.

Procedure di allarme

Gli allarmi consistono in una combinazione di allarme sonoro, luce gialla lampeggiante di STOP e messaggio descrittivo sul visualizzatore.

1. Un allarme sonoro continuo indica che l'infusione si è fermata. Premere per primo SILENZIO per silenziare l'allarme per un massimo di 2 minuti. Controllare poi se sul visualizzatore è comparso un messaggio di allarme. Premere ANNULLARE per cancellare il messaggio di allarme.

2. Quando la causa dell'allarme è stata rimossa, premere il pulsante START per riprendere l'infusione.

COMANDO DELLA POMPA DISINNESTATO - Il sistema di comando si è disinnestato durante l'infusione. Controllare l'impugnatura sagomata e la posizione della siringa.

ECCESSIVA PRESSIONE DEL SISTEMA DI COMANDO - La pressione di pompaggio ha raggiunto il limite di allarme. In seguito ad un'occlusione, il sistema di comando va in backoff (se attivato - cfr. opzioni generali) per ridurre la pressione in eccesso. In alternativa, premere le impugnature sagomate sul porta-stantuffo, per disinnestare il meccanismo di comando ed eliminare la pressione eccessiva nella siringa e nel tubo di collegamento al paziente. Accertare e rimuovere la causa del blocco nel sistema di somministrazione, prima di riavviare l'infusione.

POSIZIONAMENTO MORSETTO SIRINGA - Si è utilizzata una siringa di misura non corretta, la morsetto della siringa non è stata posizionata opportunamente oppure si è spostata la siringa durante l'operazione. Controllare la posizione della siringa, la posizione della morsetto della siringa e la siringa stessa.

STANTUFFO NON POSIZIONATO - Lo stantuffo della siringa non è inserito correttamente nel porta-stantuffo. Controllare la posizione dello stantuffo della siringa.

AVVERTIMENTO BASSO LIVELLO DI CARICA DELLA BATTERIA - Basso livello di carica della batteria con restanti massimo 30 minuti di funzionamento. L'indicatore della carica della batteria lampeggia e dopo 30 minuti un allarme sonoro continuo indica che la batteria è esaurita. Ricollegare all'alimentazione in c.a. per non interrompere il funzionamento e per ricaricare la batteria interna.

AVVERTIMENTO DI RICHIAMO INFERMIERE - L'unità è rimasta accesa per più di 2 minuti senza operazione di avvio. Premere STOP o uno degli altri pulsanti di controllo per silenziare l'allarme per altri 2 minuti. In alternativa, premere e mantenere abbassato STOP per 2 secondi per ritardare di 15 minuti il segnale di richiamo.

Allarmi senza messaggio sullo schermo

Procedure di allarme

BATTERIA SCARICA - La batteria interna è scarica. Per silenziare l'allarme, spegnere la pompa e ricollegarla all'alimentazione in c.a. Riavviare la pompa in c.a. per caricare la batteria interna.

AVVERTIMENTO PROSSIMA FINE DELLA SIRINGA

- Questa è un'operazione temporizzata e può essere impostata tra 2% - 10%. Per impostare/modificare questo valore cfr. capitolo „Opzioni generali" in questo manuale.

FINE DELL'INFUSIONE - La pompa ha raggiunto la fine dell'infusione. Circa l'1% (può essere impostato nella configurazione) del volume rimane come residuo nella siringa per minimizzare il rischio di infusione di bolle d'aria nel sistema.

AVVERTIMENTO ALIMENTAZIONE IN C.A. INTERROTTA - L'alimentazione in c.a. è stata Interrotta e la pompa sta funzionando per mezzo della batteria. Ricollegare all'alimentazione in c.a. o premere START per silenziare l'allarme e continuare a funzionare per mezzo della batteria. Sul visualizzatore lampeggia la scritta AC POWER FAIL. L'allarme scompare automaticamente se la pompa viene ricollegata in c.a.

ERRORE INTERNO DI FUNZIONAMENTO - Il sistema di allarme ha identificato un errore interno di funzionamento. Annotare il codice di errore. Togliere l'apparecchio dal servizio e farlo controllare da un Centro di assistenza qualificato.

BATTERIA IN ESAURIMENTO O ESAURITA L'ultima volta che la pompa è stata spenta la batteria era in esaurimento.Se la pompa da allora non è stata caricata può lavorare solo in AC.

11. Selezionare l'intensità di dose predefinita usando i tasti a freccia per aumentare/diminuire l'intensità visualizzata. Quando è stato selezionato il valore desiderato, premere il tasto OK.

12. Selezionare l'intensità di dose minima usando i tasti a freccia per aumentare/diminuire l'intensità visualizzata. Quando è stato selezionato il valore minimo desiderato, premere il tasto OK.

effettuando l'infusione. Per impostare una nuova portata, usare i

tasti di velocità per selezionare il nuovo valore e premere il

pulsante START per confermarlo. Avvertimento di prossima fine siringa - Imposta l'avvertimento NEOI da 1 a 15 minuti. Punto di fine infusione - Imposta la fine del volume di infusione come percentuale del

volume della siringa da 0,5% a 5,0%. KVO a EOI - Se attivato, al punto di fine infusione la pompa si mette a

funzionare alla portata KVO di 1,0 ml/h (oppure alla portata

attualmente impostata, se di valore inferiore). Se disattivato, la

pompa si ferma al punto di fine infusione. Allarme occlusione - Imposta il livello predefinito del getto tra 0 e 7. Peso - Imposta il peso del paziente tra 0,1kg e 150kg. Il valore predefinito

è 70,0kg. Portata di spurgo - Imposta la portata di spurgo tra 100 e 500 ml. Il limite superiore

evita un'eccessiva perdita di liquido durante l'operazione di

spurgo. Bolo - Abilita la somministrazione di bolo. Valore predefinito di bolo - Imposta il valore predefinito della portata di bolo tra 10 e 1200

ml/h. Valore limite portata bolo - Imposta il valore massimo ammissibile della portata di bolo. Valore limite portata - Imposta il valore massimo ammissibile della portata dell'infusione. Visualizzatore registrazione operazioni - Se attivato, è possibile accedere alla registrazione

operazioni tramite il menu opzioni. Solo registrazione ultimo paziente - Se attivato, la registrazione delle operazioni indica solo l'ultimo

paziente (da quando si è selezionato Nuovo paziente). Se

disattivato, si può visionare la registrazione completa. Chiamata infermiere - Attiva o disattiva l'opzione chiamata infermiere. Questa opzione permette all'utente di programmare il nome dell'Ospedale, del reparto o del padiglione, che compariranno durante la sequenza di inizializzazione del visualizzatore.

Attivare le siringhe

1. Premere il pulsante di alimentazione ON/OFF in posizione OFF.

2. Tenere premuto il pulsante START e premere il pulsante di alimentazione in posizione ON.

3. Sul visualizzatore principale comparirà "000". Inserire il codice di accesso "251" usando le frecce di selezione.

4. Inserire la prima cifra e premere PROSSIMO, indicato sullo schermo.

5. Quando tutto il codice è stato inserito, premere il tasto OK indicato sullo schermo.

6. Selezionare SIRINGHE ABILITATE mediante i tasti a freccia e premere il tasto OK indicato sullo schermo.

7. Usare le frecce per spostarsi nella lista di tipi e misure di siringhe disponibili. Premere il tasto MODIFIC. per abilitare/disabilitare la siringa.

8. Premere il tasto OK per memorizzare le siringhe abilitate all'uso.

9. Quando la procedura di impostazione è stata portata a termine, spegnere la pompa e rimetterla in servizio oppure selezionare la successiva opzione di configurazione.

Questa opzione è impiegata per pre-configurare il tipo e la misura della siringa idonea per l'uso da parte di un operatore. Selezionare tutte le siringhe che possono essere utilizzate con la pompa. Disabilitare tutte le siringhe che non si devono usare.

Einführung

Merkmale der IVAC®P7000 Mk II Spritzenpumpe

Die IVAC®P7000 Actilyse ist eine hochwertige Spritzenpumpe mit variablem Druck und umfangreichen Funktionen für Anwendungen in der Intensivmedizin. Die IVAC

P7000 Actilyse Spritzenpumpe mit variablem Druck gehört zur IVAC®Familie der Spritzenpumpen der "P"-Serie. Viele Aspekte der allgemeinen Funktion, der Leistungsmerkmale, der Genauigkeit und der Sicherheitssysteme der P7000 finden sich in der gesamten Pumpenpalette. Die IVAC

P7000 Actilyse arbeitet mit einer Reihe von Standard-Einwegspritzen mit Luer-Lock verschiedener Fabrikate und kann mit Größen zwischen 5 ml und 100ml verwendet werden. Wenn die Option für das Actilyse-Dosierschema aktiviert ist, kann die Pumpe alle Funktionen der IVAC

P7000 ausführen oder ein vorprogrammiertes, mehrphasiges Protokoll für die Infusion des ActilyseMedikaments durchführen. 100 mg Actilyse werden zur Infusion in 100 ml Wasser aufgelöst, was eine feste Konzentration von 1 mg/ml ergibt. Die Einstellung des Actilyse-Protokolls erfordert die Eingabe des Patientengewichts im Bereich 40 kg bis 150 kg. Nach der Bestätigung des Gewichts wird das Protokoll wie folgt berechnet.

 Phase 1. Ist immer auf 15 mg mit Maximaler Bolusrate eingestellt (normalerweise 1200 ml/h).  Phase 2. Bei Patienten mit einem Gewicht von 65 kg und mehr werden 50 mg in 30 Minuten

infundiert. Bei Patienten unter 65 kg beträgt die Dosis 0,75 mg pro kg Körpergewicht in 30 Minuten (maximal 50 mg).

 Phase 3. Bei Patienten mit einem Gewicht von 65 kg und mehr werden 35 mg in 60 Minuten

infundiert. Bei Patienten unter 65 kg beträgt die Dosis 0,5 mg pro kg Körpergewicht in 60 Minuten (maximal 35 mg).

Beispiel 1 - Patient von 83 kg.

 Phase 1 - 15 mg Bolus - Rate = Maximale Bolusrate (normalerweise 1200 ml/h).  Phase 2 - 50 mg Dosis. Bei einer vorgegebenen Konzentration von 1 mg/ml beträgt das zu

verabreichende Volumen 50,0 ml in 30 Minuten, was eine Rate von 50.0 x 2 = 100 ml/h ergibt.

 Phase 3 - Bei einer vorgegebenen Konzentration von 1 mg/ml beträgt das zu verabreichende

Volumen 35,0 ml in 60 Minuten, was eine Rate von 35.0 x 1 = 35.0 ml/h ergibt.

Beispiel 2 - Patient von 62 kg.

 Phase 1 - 15 mg Bolus - Rate = Maximale Bolusrate (normalerweise 1200 ml/h).  Phase 2 - 62 kg x 0,75 mg/kg = 46,5 mg Dosis. Bei einer vorgegebenen Konzentration von 1 mg/ml

beträgt das zu verabreichende Volumen 46,5 ml in 30 Minuten, was eine Rate von 46,5 x 2 = 93,0 ml/h ergibt.

 Phase 3 - 62 kg x 0,5 mg/kg = 31,0 mg Dosis. Bei einer vorgegebenen Konzentration von 1 mg/ml

beträgt das zu verabreichende Volumen 31,0 ml in 60 Minuten, was eine Rate von 31,0 x 1 = 31,0 ml/h ergibt.

DRUCKAUFNEHMER Erkennt, ob ein Infusionsschlauch mit Druckscheibe angeschlossen ist. Der Druckaufnehmer mißt den positiven Druck im Infusionsschlauch.

STOP-Taste - Drücken Sie "Stop", um die Infusion anzuhalten. Die gelbe Anzeige leuchtet auf, um einen Alarm anzuzeigen.

Drücken Sie FÜLLEN/BOLUS, um die Infusionsleitung während der Vorbereitung zu füllen, während die TIVA angehalten wurde oder um einen Bolus mit beschleunigter Förderrate zu geben, während eine Infusion läuft.

Taste OPTION - Drücken Sie die Taste OPTIONEN, um auf den Namen des Medikaments und andere Optionen zuzugreifen.

Verwenden Sie die Taste mit dem "Doppelpfeil" zum schnellen Verkleinern bzw. Vergrößern und den "Einfachpfeil" zum langsamen Verkleinern bzw. Vergrößern der im Display angezeigten Werte beim Vorbereiten und der Konfiguration der Pumpe. Die Tasten werden auch als (Infusions-)Ratentasten bezeichnet.

BATTERIE - Wenn diese Anzeige leuchtet, läuft die Pumpe auf Batteriebetrieb. Wenn diese Anzeige blinkt, ist die Batterie fast leer, und es verbleiben ca. 30 Minuten Betriebsdauer.

Verwenden Sie die "leeren Multifunktionstasten" in Verbindung mit den Anweisungen im Display.

DRUCK - Drücken Sie die Taste DRUCK, um den Pumpendruck anzuzeigen.

EIN/AUS - Zum Einschalten der Pumpe einmal drücken. Zum Ausschalten der Pumpe drücken und 3 Sekunden lang gedrückt halten .

NETZBETRIEB - Wenn diese Anzeige leuchtet, ist die Pumpe an das Netz angeschlossen und die interne Batterie wird geladen.

Vorsichtsmaßnahmen

Diese IVAC®-Pumpe wurde zur Verwendung mit Einwegspritzen geeicht. Um eine korrekte und genaue Funktion sicherzustellen, verwenden Sie nur diejenigen Versionen der auf der Pumpe oder in der Gebrauchsanweisung beschriebenen Spritzen mit Luer-Lock. Bei der Verwendung ungeeigneter Spritzen oder Infusionssets kann die Funktion der Pumpe und die Genauigkeit der Infusion beeinträchtigt werden.

Ein unkontrollierter Fluß oder eine Fehlförderung kann auftreten, wenn die Spritze in die Pumpe eingelegt wurde, ohne daß der Fingerflansch und der Kolben korrekt in den dafür vorgesehenen Schlitz eingelegt wurde, oder wenn die Spritze aus der Pumpe entfernt wird, bevor der Infusionsschlauch richtig vom Patienten abgetrennt wurde. Die Isolation der Infusionsleitung kann erfolgen, indem ein Hahn in der Patientenleitung geschlossen wird oder indem eine Klemme geschlossen wird.

Wenn verschiedene Apparate und/oder Geräte mit Infusionsets und anderen Infusionsleitungen z.B. über einen Dreiwegehahn kombiniert werden, kann die Leistung der Pumpe beeinträchtigt werden, und die Infusion sollte genau überwacht werden. Gegenseitige Beeinflussung kann nicht ausgeschlossen werden. Siehe: VDE 0753 Teil 5 “Anwendungsregeln für Parallelinfusion Vorstellbare Anwendungsverfahren” bzw. BBm Anwendungshinweise zur Parallelinfusion (38910004).

Bringen Sie die Pumpe nicht so in einer vertikalen Position an, daß der Wechselstromanschluß oder die Spritze nach oben zeigen, da dies die elektrische Sicherheit im Falle eines Austretens von Flüssigkeit über die Pumpe beeinträchtigen könnte, oder zur Infusion von Luft führen könnte, die sich möglicherweise in der Spritze befindet. Um eine Infusion von Luft zu verhindern, sollte der Benutzer den Fortgang der Infusion, die Spritze, den Infusionsschlauch und die Anschlüsse am Patienten regelmäßig entsprechend den hier aufgeführten Anweisungen zum Befüllen genau überprüfen.

Verschiedene, von dieser Pumpe entdeckte Alarmbedingungen, halten die Infusion an und erzeugen akustische Alarme. Die Benutzer müssen regelmäßige Kontrollen durchführen, um sicherzustellen, daß die Infusion richtig läuft und keine Alarme aufgetreten sind.

Dies ist eine mit positivem Druck arbeitende Pumpe, mit der eine sehr genaue Infusion von Flüssigkeiten erreicht werden soll, indem Widerstände im Infusionssystem automatisch kompensiert werden.

Das Pumpendruck-Alarmsystem dient nicht zum Schutz gegen oder zur Erkennung von Infiltrationen, die bei niedrigen Druckwerten auftreten können. Es besteht Explosionsgefahr, wenn die Pumpe in Anwesenheit von entflammbaren Anästhetika betrieben wird. Achten Sie darauf, daß die Pumpe von solchen Gefahrenquellen ferngehalten wird. Wenn das Gehäuse der Pumpe geöffnet oder entfernt wird, besteht die Gefahr eines Stromschlags. Lassen Sie Wartungsarbeiten nur von qualifiziertem Kundendienstpersonal durchführen. Diese Pumpe ist gegen die Auswirkungen äußerer Störungen geschützt, einschließlich starker Funkfrequenzemissionen, Magnetfeldern und elektrostatischer Entladungen (wie sie z.B. von elektrochirurgischen und Kauterisationsgeräten, großen Motoren, Kofferradios, Mobiltelefonen etc. erzeugt werden) und sie ist so konstruiert, daß sie ausfallsicher ist, falls übermäßige Störungen auftreten. Unter bestimmten Umständen kann die Pumpe durch elektrostatische Entladungen von +/-8kV (Kontakt), +/-15kV (Luft) gestört werden. Bei niedrigeren Testwerten als diesen Werten arbeitet die Pumpe normal weiter. In seltenen Fällen kann die Pumpe von Störungen mit Werten von 10V/m beeinflußt werden. Wenn die Pumpe von diesen externen Interferenzen betroffen ist, reagiert sie ausfallsicher oder führt ein Reset durch, (nach 2 Minuten wird ein Erinnerungsalarm ausgelöst). Sollten falsche Alarmbedingungen auftreten, entfernen Sie entweder die Störquelle oder regulieren Sie die Infusion auf andere Art und Weise. Diese Pumpe sendet eine bestimmte Menge elektromagnetischer Strahlung aus, die innerhalb der in IEC60601-2-24 und IEC60601-1 angegebenen Spezifikationen liegt. Wenn die Pumpe jedoch mit anderen Geräten interagiert, sollten Maßnahmen ergriffen werden, um die Auswirkungen zu minimieren, z.B. durch Umstellen der Pumpe an einen anderen Ort. Falls diese Pumpe fallen gelassen wird, zu hoher Feuchtigkeit, Luftfeuchte oder hohen Temperaturen ausgesetzt wird oder anderweitig vermutet wird, daß sie beschädigt wurde, nehmen Sie sie außer Betrieb und lassen Sie sie von einem qualifizierten Kundendiensttechniker überprüfen. Ein umfangreiches Wartungshandbuch mit Beschreibungen der Schaltkreise und Wartungsund Testinformationen ist zu dieser Pumpe erhältlich. Installation

Schließen Sie die Pumpe 24 Stunden lang an das Stromnetz an, um sicherzustellen, daß die interne Batterie vollständig geladen ist. Falls die Pumpe nicht richtig funktioniert, legen Sie sie wieder in die Original-Schutzverpackung und wenden Sie sich zwecks Untersuchung an einen qualifizierten Techniker.

Stativklemme

Die Stativklemme wird auf der Rückseite der Pumpe angebracht geliefert und dient zur sicheren Fixierung an Standard-Infusionsstativen mit einem Durchmesser bis 40 mm. Die Stativklemme kann in 4 verschiedenen Positionen angebracht werden, die es erlauben, daß sie an vertikalen und horizontalen Stativen, Geräteschienen und Krankenhausmobiliar in einer Reihe zur Bedienung bequemer Positionen angebracht werden kann. Die Stativklemme kann zur Verwendung mit horizontalen Befestigungen mit den mitgelieferten Befestigungsschrauben in den alternativen Befestigungslöchern in der Stativklemme verstellt werden. Die Stativklemme kann auch am Boden der Pumpe in vier verschiedenen Positionen befestigt werden.

Wichtige Hinweise: Bringen Sie die Pumpe nie mit dem Netzeingang oder der Spritze nach oben an. Dies kann die elektrische Sicherheit im Falle eines Auslaufens von Flüssigkeit beeinträchtigen oder zu einer Infusion von Luft führen, die sich möglicherweise in der Spritze befindet.

Ersetzen der Sicherungen für den Wechselstromanschluß

Wenn das Batteriesymbol an der Pumpe ständig leuchtet und die Wechselstromanzeige nicht aufleuchtet, auch wenn die Pumpe an das Stromnetz angeschlossen und eingeschaltet wird, muß vermutet werden, daß die internen Wechselstromsicherungen durchgebrannt sind. Es wird empfohlen, daß die Wechselstromsicherungen nur von qualifizierten Technikern gewechselt werden. Weitere Informationen zum Ersetzen der internen Wechselstromsicherungen finden Sie im technischen Wartungshandbuch.

Schutzleiter

Bitte beachten Sie, daß das Netzteil dieser Pumpe einen Schutzleiter besitzt. Der Schutzleiter soll elektrische Störungen anderer medizinischer oder elektrischer Geräte verhindern. Das Stromnetz, an das diese Pumpe angeschlossen wird, muß eine Erdung besitzen.

Wichtig:

Der Schutzleiteranschluß an dieser Pumpe ersetzt nicht den PE-Anschluß.

Laden der Spritze

Setzen Sie die Pumpe auf eine stabile, horizontale Oberfläche oder sichern Sie sie mit der angebrachten Stativklemme. Bereiten Sie die Einmalspritze vor, laden und füllen Sie diese und die Infusionsleitung unter Verwendung aseptischer Standardtechniken.

1. Drücken Sie den Lösehebel am Kolbenhalter und schieben Sie den Mechanismus nach links. Heben Sie die Spritzenklemme und drehen Sie sie nach links.

2. Führen Sie die Spritze in die Schlitze im Kolbenhalter.

3. Drücken Sie den Lösehebel am Kolbenhalter und schieben Sie den Mechanismus nach rechts, bis der Fingerflansch in den V-förmigen Schlitz eingesetzt werden kann.

4. Drehen Sie die Spritzenklemme, bis sie am Spritzenzylinder einrastet.

5. Überprüfen Sie, ob der Spritzenkolben und der Fingerflansch richtig in ihren Schlitzen sitzen.

Wichtige Hinweise: Schieben Sie die Spritze weiter ein, bis der Fingerflansch die Vorderseite des v-förmigen Schlitzes nahe der Spritzenklemme berührt. Dies ist wichtig, um eine Verzögerung beim Beginn der Infusion zu vermeiden.

Die Verwendung einer falschen Spritze kann die Genauigkeit der Infusion und die Leistung der Pumpe beeinträchtigen. Wenn Sie die Spritze füllen, berücksichtigen Sie bitte die in der Infusionsleitung und in der Spritze verbleibende Restmenge, da dieser sogenannte "TOTRAUM" nicht infundiert wird.

Wichtige Hinweise: Die Pumpe arbeitet automatisch mit der internen Batterie, wenn sie eingeschaltet wird, ohne an das Stromnetz angeschlossen zu sein. Überprüfen Sie bei jedem Einschalten der Pumpe, ob die Pumpe zweimal piept und alle Segmente des Displays und die grüne und die gelbe Anzeige während des Selbsttests aufleuchten.

Start P7000 Actilyse-Option

1. Schließen Sie die Pumpe mit dem Netzkabel an das Stromnetz an. Drücken Sie die EIN/AUS-Taste in die Position EIN. Die Pumpe läuft automatisch auf der eigenen, internen Batterie, wenn Sie eingeschaltet wird, ohne daß sie an das Netz angeschlossen ist.

2. PATIENT NEU? - Wenn Sie die Pumpe für einen neuen Patienten einstellen, antworten Sie mit JA, indem Sie die Taste unter der Anzeige JA drücken. Die Antwort JA löscht automatisch die Daten des vorherigen Patienten. Wenn Sie mit NEIN antwor- ten, bleiben die Daten des vorherigen Patienten erhalten.

3. ACTILYSE VERWENDEN? - Antworten Sie mit JA, damit der Einstellungsbildschirm für Actilyse angezeigt wird.

4. GEWICHT EINGEBEN - Geben Sie das Gewicht ein und drücken Sie OK.

5. PROTOKOLL-ANZEIGE - Überprüfen Sie das angezeigte Protokoll und drücken Sie BESTÄTIGEN, um fortzufahren oder ZURÜCK, um das Patientengewicht zu ändern. Überprüfen Sie bei der Bestätigung des Protokolls, ob Uhrzeit und Datum stimmen.

6. SPRITZE BESTÄTIGEN - Überprüfen Sie, ob das verwendete Spritzenfabrikat und die Größe mit der Anzeige im Display übereinstimmen. Falls erforderlich, kann das Spritzenfabrikat durch Drücken der Taste TYP geändert werden. Drücken Sie BESTÄTIGEN, wenn das richtige Fabrikat und die richtige Größe angezeigt werden. Das Display zeigt die aktuelle Actilyse-Phase und ACTILYSE blinkt mit IM HALT.

7. FÜLLEN (falls erforderlich) - Drücken Sie die Taste

FÜLLEN/BOLUS und dann die beiden Tasten FÜLLEN zusammen, bis die Infusion läuft und die

Infusionsleitung vollständig gefüllt ist. Der akustische Alarm ist während der Verwendung der FÜLLEN-Tasten aktiviert, und das für das Füllen verwendete Volumen wird im Display angezeigt.

8. PATIENTEN ANSCHLIESSEN - Schließen Sie die Infusionsleitung an den Zugang des Patienten an und setzen Sie die Druckscheibe in den Druckaufnehmer ein.

9. START - Drücken Sie START, um die Pumpe zu starten. Die Pumpe zeigt INFUSION und auf der rechten Seite des Displays das Fortschreiten jeder der drei Actilyse-Phasen an. Die GELBE STOP-ANZEIGE wird durch die blinkende GRÜNE START-ANZEIGE ersetzt, um anzuzeigen, daß die Pumpe läuft.

10. STOP - Nach Beendigung des Actilyse-Protokolls wird die Infusion gestoppt. Das Display zeigt das insgesamt infundierte Volumen an. Die Pumpe kehrt zur normalen Betriebsart zurück.

Wenn die Actilyse-Option aktiviert ist, unterscheidet sich die Bedienung der P7000 auf folgende Art und Weise:

 Eine FESTE RATE ist permanent eingestellt und unabhängig von der Einstellung der

RATE FREIGEBEN in den allgemeinen Optionen wird keine Option angegeben, die Rate direkt zu verändern.

 Eine Ratentitrierung ist niemals möglich, unabhängig von der Einstellung der RATEN-

TITRIERUNG in den allgemeinen Optionen.

 Eine Bolusgabe ist während des Actilyse-Betriebs nicht gestattet.

Unterschiede in der Bedienung der P7000 mit Actilyse-optionen

Füllen

Die Füllfunktion kann nicht verwendet werden, wenn die Funktion "Rate fest einstellen" aktiviert ist.

EIN/AUS

Drücken Sie zum Einschalten der Pumpe die Taste EIN/AUS einmal.

Drücken Sie zum Ausschalten der Pumpe die Taste EIN/AUS und halten Sie sie 3 Sekunden lang gedrückt. Das Display zählt herunter und die Pumpe wird ausgeschaltet.

1. Um die Füllfunktion zu verwenden, halten Sie die Pumpe an, und drücken Sie die Taste FÜLLEN.

2. Das Display ändert sich und zeigt zwei Fülltasten im Display an. Drücken Sie zum Füllen beide Tasten gleichzeitig. Das Füllvolumen wird nicht zum infundierten Volumen hinzugerechnet.

3. Wenn die Füllfunktion abgeschlossen ist, drücken Sie bitte die Multifunktionstaste ABBRUCH, um das Menü zu verlassen.

Starten der Pumpe - Normal-Betriebsart

1. Schließen Sie die Pumpe mit Hilfe des Netzkabels an eine Steckdose an. Stellen Sie die EIN/AUS-Taste in die Position EIN.

2. NEUER PATIENT? - Wenn Sie mit NEIN antworten, bleiben alle Patientendaten erhalten. Wenn Sie mit JA antworten, werden die Patientendaten automatisch gelöscht. Überprüfen Sie, ob die richtige Uhrzeit und das richtige Datum erscheinen. Die Frage wird übergangen, wenn AUTOSAVE ausgeschaltet ist. Alle Patientendaten werden gelöscht.

3. SPRITZE LADEN - Laden Sie die Spritze in Übereinstimmung mit der entsprechenden Anweisung im Abschnitt "Bedienung" dieser Gebrauchsanweisung.

4. SPRITZE BESTÄTIGEN - Überprüfen Sie, ob das verwendete Spritzenfabrikat und die Größe mit der Anzeige im Display übereinstimmen. Falls erforderlich, kann das Spritzenfabrikat durch Drücken der Taste TYP geändert werden. Drücken Sie BESTÄTIGEN, wenn das richtige Fabrikat und die richtige Größe angezeigt werden.

5. Überprüfen Sie bitte die angezeigte Rate, wenn die alten Patientendaten verwendet werden, und ändern Sie sie, falls erforderlich, mit Hilfe der EINSTELLEN- Tasten.

6. FÜLLEN (falls erforderlich) - Drücken Sie die Taste FÜLLEN/BOLUS; und dann die beiden Tasten FÜLLEN zusammen, bis die Infusion läuft und die Infusionsleitung vollständig gefüllt ist. Der akustische Alarm ist während der Verwendung der FÜLLEN- Tasten aktiviert, und das für das Füllen verwendete Volumen wird im Display angezeigt.

7. PATIENTEN ANSCHLIESSEN - Schließen Sie die Infusionsleitung an den Zugang des Patienten an.

8. START - Drücken Sie START, um die Pumpe zu starten. Die Pumpe zeigt INFUSION an. Die GELBE STOPANZEIGE wird durch die blinkende GRÜNE STARTANZEIGE ersetzt, um anzuzeigen, daß die Pumpe läuft.

Füllen

EIN/AUS

Bolusinfusionen

1. Drücken Sie während der Infusion die Taste

BOLUS einmal, um den Bolus-Bildschirm anzuzeigen.

2. Verwenden Sie die Pfeiltasten, um die erforderliche Bolusdosis einzustellen.

3. Drücken Sie gleichzeitig die beiden BOLUS- Multifunktionstasten, um den Bolus abzugeben. Während des Bolus zeigt die Pumpe das infundierte Volumen an.

4. Lassen Sie die beiden BolusMultifunktionstasten los, wenn der gewünschte Bolus abgegeben wurde. Das Bolusvolumen wird zum insgesamt infundierten Volumen addiert. Drücken Sie ABBRUCH, um den Bolusbildschirm zu verlassen.

Die Bolusfunktion kann sowohl während der Induktion als auch während der Infusion verwendet werden. Wenn das Volumen des Bolus die Bolusvolumengrenze erreicht, wird der Bolus gestoppt, und die Pumpe schaltet automatisch auf die Infusion mit der eingestellte Rate um. Wenn das zu infundierende Volumen während eines Bolus erreicht wird, wird der Alarm "Zu infundierendes Volumen erreicht" ausgelöst, und die Pumpe kehrt zum vorherigen Status zurück. Drücken Sie STILL, um den Alarm stummzuschalten, oder ABBRUCH, um den Alarm zu bestätigen.

Wichtiger Hinweis: Während der Bolusfunktion werden keine Alarme deaktiviert, Druckalarme werden temporär auf Maximalwerte gesetzt. Ein Bolus kann nicht abgegeben werden, wenn die Funktion "Rate fest einstellen" aktiviert ist, oder wenn ein Protokoll mit mehreren Dosen verwendet wird.

1. Um die Füllfunktion zu verwenden, halten Sie die Pumpe an, und drücken Sie die Taste FÜLLEN.

Wenn Protokoll verwendet wird, steht hinter dem Medikamentennamen ein *. Die Option "Einstellung in ml/h" / "Einstellung als Dosisrate" ist nun verfügbar.

Diese Optionen ermöglichen es, mit der Pumpe ein spezielles Medikamenten- und/oder Dosierungsprotokoll zu verwenden. Die Medikamente sind vorkonfiguriert (siehe Konfigurationsoptionen), um eine schnelle Auswahl des Medikamentennamens, der Dosiseinheiten und der Vorgabe für die Förderrate zu ermöglichen. Zur Erhöhung der Sicherheit werden konfigurierte maximale und minimale Sicherheitsgrenzen für die Medikamente verwendet, die hinsichtlich Konzentration und Dosisrate programmierbar sind.

Medikamente und Dosierung - Verwendung vorkonfigurierter Dosierungsprotokolle

1. Drücken Sie die Taste ?, um auf das Optionsmenü zuzugreifen.

2. Wählen Sie die Option MEDIKAMENTE UND DOSIERUNG mit Hilfe der Pfeiltasten.

Achtung:

Die Anzeige für die infundierte Masse ist eine Summe aus allen infundierten Medikamentenmassen, die aus verschiedenen Konzentrationen und sogar Arten von Medikamenten bestehen kann. Daher sollte beachtet werden, daß das Verhältnis zwischen aktuell angezeigtem infundierten Volumen und infundierter Masse sich möglicherweise nicht auf die aktuelle Konzentration bezieht.

Diese Option ermöglicht es Ihnen, ein bestimmtes zu infundierendes Volumen einzugeben und die Infusionsrate, wenn das Volumen infundiert wurde: Stop, 1 ml/h, 2 ml/h und kontinuierliche Infusion mit der eingestellten Rate.

Zu infundierendes Volumen (ZIV)

ZIV

ZIV über die Zeit

Die Option 'ZIV über die Zeit' muß in den Konfigurationsoptionen aktiviert sein. Diese Option ermöglicht die Eingabe eines ZIV und eines Abgabezeitraums. Die zur Abgabe des benötigten Volumens erforderliche Rate im erforderlichen Zeitraum wird berechnet und angezeigt. Die Infusion muß gestoppt werden, damit diese Option gewählt werden kann.

1. Drücken Sie die Taste "?", um auf das

Optionsmenü zuzugreifen.

2. Wählen Sie die Option ZIV ÜBER DIE ZEIT EINSTELLEN mit Hilfe der Pfeiltasten und drücken Sie die auf dem Bildschirm angezeigte Multifunktionstaste OK.

3. Stellen Sie das zu infundierende Volumen mit Hilfe der Pfeiltasten ein. Wenn das zu infundierende Volumen eingegeben wurde, drücken Sie die Multifunktionstaste OK.

4. Geben Sie den Zeitraum ein, über den das Volumen infundiert werden soll. Die Infusionsrate wird automatisch berechnet. Drücken Sie die Multifunktionstaste OK, um den Wert einzugeben.

5. Wählen Sie die nach Ende des ZIV zu verwendende Rate und drücken Sie die Multifunktionstaste OK. Die Grundeinstellung ist STOP.

1. Drücken Sie die ZIV-Multifunktionstaste, um die ZIV-Option auszuwählen.

2. Geben Sie das zu infundierende Volumen mittels der Ratentasten ein.

3. Wenn das zu infundierende Volumen eingegeben wurde, drücken Sie bitte die Multifunktionstaste OK.

4. Wählen Sie die Rate am Ende des zu infundierenden Volumens mittels der Ratentasten. Die Grundeinstellung ist STOP.

5. Drücken Sie die Multifunktionstaste OK, um das ZIV-Menü zu verlassen.

6. Sobald die Pumpe das eingestellte Volumen gefördert hat, gibt sie Alarm. Durch Drücken der Taste "ABBRUCH" wird der Alarm gelöscht.

7. Drücke die Taste "LÖSCHEN" um das ZIV auszuschalten, oder stelle ein ZIV mit den Pfeiltasten ein.

Einstellen Dosisrate / Einstellen Infusionsrate

Um die Raten genau in Dosisraten- oder Infusionsratenschritten einzustellen, kann es erforderlich sein, zwischen den Rateneinstellungsoptionen EINSTELLEN DOSISRATE und EINSTELLEN ml/h umzuschalten. Ein Pfeil links der Ratenanzeige zeigt die Rate an, die verändert wird, wenn die Ratentasten verwendet werden, um die Infusionsrate zu erhöhen bzw. zu verringern. Um eine Dosisrate genau einzustellen, muß der Pfeil auf die Dosisrate (mg/kg/h) zeigen. Die Infusionsrate wird mit Hilfe der Dosisrate berechnet. Um eine Infusionsrate genau einzustellen, muß der Pfeil auf die Infusionsrate (ml/h) zeigen. Die Dosisrate wird mit Hilfe der Infusionsrate berechnet.

Auswahl der Option Einstellen Dosisrate

1. Drücken Sie die Taste ?, um das

Optionsmenü aufzurufen.

2. Wählen Sie die Option EINSTELLEN DOSISRATE mit Hilfe der Pfeiltasten und drücken Sie die Multifunktionstaste ÄNDERN wie auf dem Bildschirm angegeben. Dadurch wird die Option EINSTELLEN DOSISRATE ausgewählt, der Pfeil auf dem Display wird automatisch die Dosisrate auswählen, und die Dosisrate kann eingestellt werden, falls dies erforderlich ist.

Auswahl der Option Einstellen ml/h

1. Drücken Sie die Taste ?, um das Optionsmenü aufzurufen.

2. Wählen Sie die Option EINSTELLEN FÖRDERRATE mit Hilfe der Pfeiltasten und drücken Sie die Multifunktionstaste ÄNDERN, wie auf dem Bildschirm angegeben. Induktion

Die Induktionsoption wird innerhalb der allgemeinen Optionen aktiviert. Siehe Allgemeine Optionen in dieser Gebrauchsanweisung.

2. Wählen Sie die Option INDUKTION mit Hilfe

4. Stellen Sie die Induktionsrate mit Hilfe der Pfeiltasten ein und drücken Sie dann die Multifunktionstaste OK.

5. Wählen Sie das Induktionsvolumen mit Hilfe der Pfeiltasten und drücken Sie dann die Multifunktionstaste OK. Falls ein falscher Wert eingegeben wurde, drücken Sie die Multifunktionstaste ZURÜCK, um zum vorherigen Schritt zurückzukehren.

6. Geben Sie die Erhaltungsinfusionsrate mit Hilfe der Pfeiltasten ein und drücken Sie dann die Multifunktionstaste OK.

7. Überprüfen Sie die Induktionsdaten auf dem Bildschirm und drücken Sie dann BESTÄTIGEN. Falls erforderlich, drücken Sie die Multifunktionstaste ZURÜCK, um zu den Daten zurückzukehren. Wenn die Daten bestätigt werden, kehrt die Pumpe zum Hauptdisplay zurück.

8. Die Induktion kann gelöscht werden. Stoppen Sie die Infusion und drücken Sie dann die Taste ?. Wählen Sie mit Hilfe der Pfeiltasten die Option INDUKTION LÖSCHEN und drücken Sie die Multifunktionstaste OK, um die Eingabe zu bestätigen.

6. Geben Sie die Erhaltungsinfusionsrate mit Hilfe der Pfeiltasten ein und drücken Sie dann die Multifunktionstaste OK.

7. Wählen Sie mit Hilfe der Pfeiltasten die HÄUFIGKEIT (h:min) jeder Multidosis und drücken Sie dann die Multifunktionstaste OK.

8. Überprüfen Sie die Multidosisdaten auf den Bildschirm und drücken Sie dann BESTÄTIGEN. Falls erforderlich, drücken Sie die Multifunktionstaste ZURÜCK, um zu den Daten zurückzukehren. Wenn die Daten bestätigt werden, kehrt die Pumpe zum Hauptdisplay zurück. Der Abschnitt des Bildschirms für die Überprüfung zeigt entweder das verbleibende Multidosisvolumen oder die Zeit zwischen den restlichen Multidosisgaben.

Multidosis

Die Multidosisfunktion wird innerhalb der allgemeinen Optionen aktiviert. Siehe Allgemeine Optionen in dieser Gebrauchsanweisung.

Diese Option ermöglicht eine Überprüfung des 24-Log des infundierten Volumens.

der Pfeiltasten und drücken Sie die auf dem Display angezeigte Multifunktionstaste OK.

3. Drücken Sie die auf dem Display

Die Funktion Feste Rate kann im Rahmen der allgemeinen Optionen der Pumpe aktiviert werden. Wenn die Infusionsrate eingegeben und die Infusion gestartet wurde, erscheint der Hinweis auf die Feste Rate im Hauptmenü. Wenn eine Feste Rate eingegeben wurde, kann die Rate nicht verändert werden, wenn die Pumpe angehalten wird. Die Infusion kann auf normale Art und Weise gestartet und gestoppt werden. Die Funktion Feste Rate deaktiviert die Bolus- und die Füllfunktion.

1. Drücken Sie die Multifunktionstaste JA, um

die Funktion Feste Rate zu wählen. Drücken Sie die Multifunktionstaste NEIN, wenn die Eingabe einer Festen Rate nicht notwendig ist.

PUMPENANTRIEB LOSE - Das Antriebssystem hat sich während des Betriebs gelöst. Überprüfen Sie den Fingerflansch und die Position der Spritze.

ZU HOHER FÖRDERDRUCK - Der Förderdruck hat die Alarmgrenze erreicht. Nach einem Verschluß fährt der Antrieb zurück (falls die entsprechende Funktion aktiviert ist - siehe allgemeine Optionen), um überschüssigen Druck zu reduzieren. Sie können auch den Lösehebel am Kolbenhalter drücken, um den Antriebsmechanismus zu lösen und überschüssigen Druck in der Spritze und der Patientenleitung zu reduzieren. Identifizieren und beseitigen Sie die Ursache der Blockierung im Infusionssystem, bevor Sie die Infusion neu starten.

POSITION DER SPRITZENKLAMMER - Es wurde eine falsche Spritzengröße verwendet, die Spritzenklemme wurde nicht richtig plaziert oder während des Betriebs verschoben. Überprüfen Sie die Position der Spritze bzw. der Spritzenklemme und die Spritze selbst.

KEIN SPRITZENKOLBEN ERKANNT - Der Spritzenkolben sitzt nicht richtig im Spritzenkolbenhalter. Überprüfen Sie die Position des Spritzenkolbens.

WARNUNG BEI GERINGER BATTERIESPANNUNG Batterie erschöpft, noch 30 Minuten Funktionsdauer. Batterieanzeige blinkt, und nach 30 Minuten zeigt ein Dauerton an, daß die Batterie leer ist. Zum weiteren Betrieb Pumpe an Stromnetz anschließen und interne Batterie laden.

Vorgehen bei Alarm

Alarme werden durch eine Kombination von akustischen Signalen, einer blinkenden gelben STOP-Anzeige und einer beschreibenden Meldung im Display angezeigt.

1. Ein anhaltender Alarmton zeigt an, daß die Infusion unterbrochen wurde. Drücken Sie zuerst die Taste STILL, um den Alarmton für maximal 2 Minuten zu unterdrücken, und überprüfen Sie dann das Display auf eine Alarmmeldung. Drücken Sie AUS, um die Alarmmeldung zu unterdrücken. Ein unterbrochener Alarmton zeigt einen Alarm an, der die Infusion nicht unterbrochen hat.

2. Wenn die Ursache für den Alarm beseitigt wurde, drücken Sie die Taste START, um die Infusion wieder aufzunehmen.

BATTERIE LEER - Interne Batterie erschöpft. Zum Abstellen des Alarms Pumpe ausschalten und an Stromnetz anschließen. Pumpe weiter mit Netzstrom betreiben, während interne Batterie geladen wird, und auf BETRIEB schalten.

WARNUNG VOR FAST LEERER SPRITZE - Dieser Alarm kann zeitlich definiert werden und zwischen 1 Minute und 15 Minuten eingestellt werden. Zum Einstellen bzw. Ändern dieses Werts siehe Abschnitt Optionen in dieser Gebrauchsanweisung.

SPRITZE LEER - ENDE DER INFUSION - Die Pumpe hat das Ende der Infusion erreicht. Ca. 1% des Spritzenvolumens (kann bei der Konfiguration eingestellt werden) verbleibt in der Spritze, um die Infusion von Luftblasen in das Infusionsbesteck zu vermeiden. Wenn die Venenoffenhaltefunktion (KVO) am Ende der Infusion aktiviert ist, fällt die Rate auf die KVO-Rate ab. Zirka die Hälfte des in der Spritze verbleibenden Volumens wird mit der KVO-Rate abgegeben.

WARNUNG BEI GEZOGENEM NETZSTECKER - Der Netzstecker wurde gezogen, und die Pumpe läuft auf Batteriebetrieb. Stecken Sie den Netzstecker wieder ein, oder drücken Sie START, um den Alarm auszuschalten, und mit dem Batteriebetrieb fortzufahren. Im Display blinkt BATTERIEBETRIEB. Der Alarm wird automatisch gelöscht, wenn der Netzstecker wieder eingesteckt wird. WICHTIGER HINWEIS: Wenn der Netzspannungsalarm ausgeschaltet wird, löst das Abziehen des Netzkabels keinen Alarm aus.

INTERNE FEHLFUNKTION - Das Alarmsystem hat eine interne Fehlfunktion entdeckt. Notieren Sie den Fehlercode. Pumpe außer Betrieb nehmen und durch qualifizierten Techniker überprüfen lassen.

BATTERIE NIEDRIG ODER LEER: Beim letzten Ausschalten der Pumpe war die Batteriekapazität gering. Wurde die Batterie seitdem nicht geladen, muß mit Netzkabel gearbeitet werden.

1. Drücken Sie die EIN/AUS-Taste in die Stellung AUS.

2. Halten Sie die Taste START gedrückt und drücken Sie die EIN/AUS-Taste in die Stellung EIN.

3. Das Hauptdisplay zeigt "000". Geben Sie mit Hilfe der Pfeiltasten den Zugangscode "251" ein.

4. Geben Sie die erste Zahl ein und drücken Sie die auf dem Bildschirm angezeigte Taste WEITER.

5. Wenn der vollständige Code eingegeben wurde, drücken Sie die auf dem Bildschirm angezeigte Multifunktionstaste OK.

6. Wählen Sie mit Hilfe der Pfeiltasten die Option MEDIK EINSTELLEN und drücken Sie die auf dem Bildschirm angezeigte Multifunktionstaste OK.

7. Wählen Sie mit Hilfe der Pfeiltasten das benötigte Medikament aus und drücken Sie die Multifunktionstaste OK.

8. Um ein Medikament zu verwenden, muß es freigegeben werden. Drücken Sie die auf dem Bildschirm angezeigte Multifunktionstaste JA, um das gewählte Medikament zu aktivieren.

9. Um einen Medikamentennamen hinzuzufügen oder zu ändern, verwenden Sie die Pfeiltasten, um das Alphabet durchzugehen. Drücken Sie die Multifunktionstaste WEITER, um einen Buchstaben auszuwählen. Wenn Sie fertig sind, drücken Sie die auf dem Bildschirm angezeigte Multifunktionstaste OK.

10. Wählen Sie die für die Erhaltungsdosis erforderlichen Dosiseinheiten. Verwenden Sie die Pfeiltasten, um die erforderlichen Dosiseinheiten auszuwählen. Wenn Sie die Dosiseinheiten ausgewählt haben, drücken Sie die Multifunktionstaste OK.

11. Wählen Sie die Vorgabe der Dosisrate und verwenden Sie die Pfeiltasten, um die angezeigte Dosisrate zu erhöhen bzw. zu verringern. Wenn Sie die erforderliche Dosisrate ausgewählt haben, drücken Sie die Multifunktionstaste OK.

12. Wählen Sie die Minimale Dosisrate und verwenden Sie die Pfeiltasten, um die angezeigte Dosisrate zu erhöhen bzw. zu verringern. Wenn Sie die Minimale Dosisrate ausgewählt haben, drücken Sie die Multifunktionstaste OK.
