An individual release is required for each combination of LITHOSTAR MODULARIS Vario
with an X-ray C-Arm unit. Prerequisite is a complete startup specific to the Serial Numbers
of the units.
If a LITHOSTAR MODULARIS Vario is operated with more than one C-Arm unit, a separate Release Certificate must be generated for each combination.

The response with the signed release must be filed in the System Binder that is with the
user. Please file a copy of the Dimensional Protocol along with it in the PG
(SPL1-130.891.01).

The combination described above is hereby released for shock wave operation. This
release applies for the technical version on the day of turnover to the customer. It is no
longer valid if changes are made to the technical operating requirements.
