End-user consumer purchasers who purchase qualifying new LG SIGNATURE models at participating retailers in the 50 U.S. states, D.C., or P.R. during the Program Period (deﬁned below) will be eligible to receive the speciﬁed LG Visa Prepaid Card by mail-in rebate subject to the terms and conditions below. For the purposes of this promotional offer, “Qualifying new LG SIGNATURE models” includes the models outlined on the front side of this rebate form. Except as otherwise required by law, this promotional offer is limited to one LG Visa Prepaid Card per household on each qualifying new LG SIGNATURE purchase made between July 30, 2020 and September 30, 2020 (the “Program Period”). Consumers cannot claim multiple rebates based on the purchase of the same product. The maximum number of products in the same category allowed to qualify for this rebate is 1. Purchase date is determined by receipt. Purchase of qualifying new LG SIGNATURE models during the Program Period must appear on the same sales receipt. Consumers submitting claims by mail must submit the following in one envelope: (1) original, dated sales receipt of eligible products purchased during the program period or legible copies of original dated sales receipt from participating retailers dated between July 30, 2020 and September 30, 2020 (2) redemption form, completely and legibly ﬁlled out.
