All entries must be accompanied by this form, filled in entirely with all requested information. Incomplete entry forms will be disqualified. All entries must be received by January 16, 2004 to be eligible.

• Floor plan and elevation or perspective

drawings in 8

/2 "x 11" format and minimum of two color photographs (5"x 7" or 8"x 10") of complete kitchen.

• Narrative statement of no more than 200 words

describing how Sub-Zero and Wolf products benefitted the kitchen design, as outlined in the official contest rules. Include any unusual client needs or requirements.
