IMPORTANT INFORMATION: The actual serial tag from the unit (rather than the box sku or a copy of the serial tag) must be removed from the appliance, affixed to a sheet of paper and attached to the RTM form. Please mail the RTM form, the serial tag and this form to the address below. To avoid a delay or denial of your credit request, please be sure all required documentation is included.

Below are the required documents that must be submitted in order to obtain a credit on the defective unit, along with a summary of documentation already on file (if any). Please remember the unit must have been in the consumer’s home for 7 or less days to qualify for credit.
