WARNING
ELECTRICAL HAZARD!
BE SURE POWER IS OFF!
• 1-1/2” Deep Floor Box and Cover Assembly.
• Use only on wood floors over concrete decking or slab.
• Not for use on tile covered floors.
• 1/8” to 1/4” thick vapor/sound barrier over concrete.
• Subfloor must be a minimum height of 1” thick.
• Finished floor must be a minimum thickness of 1/2”.
• Total sub-floor and finished floor thickness of 1-1/2” min / 1-9/16” max required for box and cover installation.

Figure 3 shows a Section View with minimum thickness of
wood subfloor and finished floor.

1.
Cut subfloor as shown in Figure 1.
2.
Use box (G) as template and drill two 5/32” pilot holes through
vapor barrier into concrete to attach box.
3.
Secure box (G) to concrete using two concrete tapping
screws (O).
4. Break ears off duplex (See Detail A).
5.
Wire duplex receptacle and run conduit per local and national
electrical codes.
6. Place duplex seating plate (R) on top of duplex (not furnished)
and secure to mounting plate (H) using screws and nuts (L) (M).
7. Use ground screw and grounding washer provided (P) (Q) to
ground duplex to box.

Secure cover/duplex mounting plate (H) to box using two
screws (N).

1.
Install or cut finished floor around floor box as shown in Figure 1.
2.
Place gasket (B) over duplex and mounting plate (H) (Figure 1).
3.
Apply a moderate amount of latex caulk or sealant directly to
finished floor fully around square hole and gasket (B) (Figure 1).
4.
Attach cover (A) to box over gasket (B) using screws (D). Use
latex caulk or sealant under screw heads to block water entry.
5.
Clean excess sealant from around cover edge (Figure 1 and 2).
6.
Use thread sealer on cover plugs (C) to block water entry.
