THE FOLLOWING STEPS MUST
BE PERFORMED WHEN LOCKING
OUT EQUIPMENT:

1 - Identify all potential hazards
and energy sources.
2 - Inform operator / supervisor about
work to be performed.

3 - Prepare equipment for shutdown.

4 - Shutdown / turn off equipment.

5 - Isolate and LOCKOUT energy sources.

6 - Release all stored energy.

7 - Test equipment for zero e

8 - Clear personnel / tools before
and after job.
9 - Remove locks and tags when work
completed.
