Always perform an Electrical De-Energization Test and Blocking Verification Test before
using this product.

ELECTRICAL DE-ENERGIZATION TEST:

All associated circuitry and /or energy isolating devices should be electrically verified as de­energized prior to and following the installation of the lockout devices.

BLOCKING VERIFICATION TEST:

If this lockout device cannot be affixed in a manner that will hold the appropriate energy
isolating devices in a safe or off position, the device SHALL NOT be used.
APPPROPRIATE USE of these lockout devices is the sole responsibility of the user in
relation to compliance with Lockout Tagout regulations/standards and Company Safety
procedures.
