2. Description
The Affinity NT™ integrated Trillium™ cardiotomy/venous reservoir (CVR)/oxygenator with Trillium biosurface is
a single-use gas exchange device with integral stainless steel heat exchanger and cardiotomy/venous reservoir.
The primary blood contact surfaces of the CVR and the oxygenator are coated with Trillium biosurface (a
polymer coating containing nonleaching heparin)1. Venous blood is collected and defoamed while cardiotomy
blood is collected, defoamed, and filtered before mixing with the venous blood. The venous blood is pumped
through the heat exchanger for temperature regulation and then outside the hollow fibers in the membrane
compartment for gas exchange.

3. Indications for use
The Affinity NT integrated Trillium CVR/oxygenator with Trillium biosurface is intended to be used in an
extracorporeal perfusion circuit to collect venous and cardiotomy suctioned blood, cool or warm the blood, and
oxygenate and remove carbon dioxide from the blood during routine cardiopulmonary bypass (CPB) procedures
up to 6 hours in duration.
4. Contraindications
Use the device only as indicated.
5. Warnings
Read all warnings, precautions, and instructions for use carefully prior to use. Failure to read and follow all
instructions, or failure to observe all stated warnings, could cause serious injury or death to the patient.
■
The fluid pathway is sterile and nonpyrogenic. Inspect each package and device prior to use. Do not use the
device if the package is opened or damaged, the device is damaged, or the protective caps are not in place.
■
If air bubbles and/or leaks are observed during priming and/or operation, these conditions may result in air
embolism to the patient and/or fluid loss. The extracorporeal circuit must be continually monitored. Do not
use the device if these conditions are observed.
■
Clear all gas emboli from the extracorporeal circuit before initiating bypass. Gas emboli are hazardous to the
patient.
■
In any extracorporeal circuit, the use of a prebypass and arterial line filter should be considered.
■
Monitoring of preoxygenator circuit pressure should be considered.
■
The blood phase pressure must be greater than the gas phase pressure at all times in the oxygenator.
■
Do not exceed blood phase pressure of 750 mmHg.
■
Do not exceed heat exchanger water phase pressure of 206.8 kPa (30 psi).
■
Heat exchanger water temperature shall not exceed 42°C (107°F).
■
Follow institution CPB protocol when using disinfectants in the heater/cooler during bypass. The integrity of
the water path has been verified with hydrogen peroxide (330 ppm). Contact Medtronic for information
regarding the use of additional disinfectants.
■
Band all tubing connections for added protection against high fluid pressures.
■
A replacement oxygenator should be readily available during perfusion.
■
Do not obstruct gas outlet vents to prevent the gas side pressures from exceeding blood side pressures.
■
The use of a blood level sensing mechanism during operation of this device is recommended.
■
Ensure that the venous reservoir outlet is always positioned above the highest point in the membrane
compartment of the Affinity NT oxygenator.
■
Do not use a vacuum source in excess of 150 mmHg suction through this device.
■
Do not occlude or obstruct the vent port of the CVR during operation.
■
Only persons thoroughly trained in CPB procedures should use this device. Operation of each device
requires constant supervision by qualified personnel for patient safety. Perfusion must be carefully and
constantly monitored.
■
Each device is intended for single use only. Do not reuse or resterilize. Sterilized using ethylene oxide.
■
Attach tubing in such a manner as to prevent kinks or restrictions that may alter blood, water, or gas flow.
■
Do not allow alcohol, alcohol-based fluids, anesthetic liquids (such as isoflurane), or corrosive solvents
(such as acetone) to come into contact with the device as they may jeopardize the structural integrity.
6. Precautions
■
This device was designed for single patient use only. Do not reuse, reprocess, or resterilize any component
of the system. Reuse, reprocessing, or resterilization may compromise the structural integrity of the device
and/or create a risk of contamination of the device, which could result in patient injury, illness, or death.
■
Use aseptic technique in all procedures.
■
Follow a strict anticoagulation protocol and routinely monitor anticoagulation during all procedures.
Adequate anticoagulation must be maintained before and during bypass.
■
Additional warnings and precautions applicable to specific procedures can be found at appropriate places in
the instructions for use.
■
It is the responsibility of the user to dispose of the devices in accordance with local regulations and hospital
procedures.
■
This product contains phthalates.

8. Information for products with Trillium biosurface
The primary blood-contacting surfaces of the product are coated with Trillium biosurface. This coated surface
enhances blood compatibility and provides a blood-contacting surface that is thromboresistant. Trillium
biosurface contains nonleaching heparin derived from porcine intestinal mucosa2.
Caution: Product coated with Trillium biosurface is intended for single use only. Resterilization may adversely
affect the Trillium biosurface.
Caution: Follow a strict anticoagulation protocol and routinely monitor anticoagulation during all procedures.
9. Instructions for use
9.1. System setup

1. Remove the devices from the packaging carefully to ensure sterile fluid pathway.
Warning: Ensure that aseptic technique is used during all stages of setup and use of this system.
Warning: Before removing the devices from the packaging, inspect the packaging and product for damage.
If the package or product is damaged, do not use as sterility of the device may have been compromised
and/or performance may be affected.
2. Place the integrated CVR and oxygenator securely into its holder.
Note: The Affinity NT oxygenator and CVR may be separated and utilized with separate holders. To
separate the oxygenator from the CVR, turn the oxygenator approximately 1/8 of a turn in a counterclockwise direction and place in their respective holders.
3. Remove the obturator cap from the positive/negative pressure relief valve.
4. Connect the 1.3 cm (1/2 in) water lines to the inlet and outlet ports on the heat exchanger. Start the heat
exchanger water flow, and check for leaks from the water compartment to the blood compartment. Discard
the oxygenator if water is present in blood compartment, and set up the standby oxygenator.
5. Connect all blood and gas lines to their appropriate connections. Do not occlude tubing in pump raceway.
6. Clamp the tubing at the oxygenator inlet, oxygenator arterial outlet, and oxygenator recirculation line outlet.

Note: A CO2 flush of the oxygenator is not required, but is recommended before priming.
9.2. Priming

1. Fill the CVR with 1.5 to 2 L of priming solution.
2. Remove all bubbles in the section of tubing from the CVR outlet to the clamp by the oxygenator inlet.
3. Remove the clamp from the oxygenator inlet, and allow solution to gravity prime the heat exchanger and
fiber bundle of the oxygenator.
Note: Albumin has been reported to reduce the rare instance of transient high inlet pressure phenomenon in
membrane oxygenators. Therefore, albumin in the prime solution should be considered.
9.3. Recirculation

1. After placing tubing into a properly occluded pump raceway, remove clamp from recirculation outlet
per Figure 9 to allow recirculation between the oxygenator and venous reservoir.
2. Begin recirculation flow at 500 mL/min and gradually increase flow to 5 L/min.
3. Ensure that oxygenator is bubble-free.
4. Gradually reduce flow, stop the pump, and clamp the recirculation line.
9.4. Priming A/V circuit

1. Add additional prime solution to fill the rest of the perfusion circuit if needed. Unclamp the arterial outlet;
slowly pump prime solution into the remaining circuit. Purge air from the arterial sample port and access port
at this time.
2. The arterial filter should now be primed per manufacturer's instructions.
3. Inspect the entire circuit and all components for leaks.
4. Upon completion of priming and debubbling, gradually reduce blood flow and stop the pump, close all purge
lines, and clamp the arterial, venous, and recirculation lines.
5. Ensure that the entire extracorporeal circuit is free of bubbles before initiating bypass.
9.5. Initiation of bypass
Warning: The blood phase pressure must be greater than the gas phase pressure at all times.
Note: Priming solution may be prewarmed through the heat exchanger before initiation of bypass.
1. Check for adequate levels of anticoagulation before initiation of bypass.
2. Remove the arterial and venous clamps and gradually increase blood flow. Then initiate gas to blood flow
ratio of 1:1 with a minimum FiO2 of 80%.
3. Initiate water flow through the heat exchanger.
Warning: Do not allow air to enter the circuit.
9.6. Operation during bypass
1. Arterial PO2 is controlled by varying the percent concentration of oxygen present in the ventilating gas.
■
To decrease the PO2, decrease the amount of oxygen in the ventilating gas by decreasing the FiO2 on
your oxygen blender.
■
To increase the PO2, increase the amount of oxygen in the ventilating gas by increasing the FiO2 on
your oxygen blender.
2. PCO2 is controlled primarily by varying total gas flow rate.
■
To decrease PCO2, increase the total gas flow rate to increase the amount of CO2 removed.
■
To increase PCO2, decrease the total gas flow rate to decrease the amount of CO2 removed.
3. Patient temperature is controlled by regulating the temperature of water flow into the heat exchanger.
Note: Care should be taken to minimize temperature gradient during cooling and rewarming phases of bypass.
9.7. Termination of bypass
1. Terminate bypass as individual case and patient condition indicate.
2. Turn off gas flow before turning off blood flow.
3. Turn off water flow to heat exchanger.
4. After terminating bypass, continue to slowly recirculate through oxygenator using recirculation line and/or
reconnected arterial venous loop.
9.8. Emergency oxygenator replacement
A standby Affinity NT integrated Trillium CVR/oxygenator with Trillium biosurface should always be available
during perfusion.
1. Turn off water flow to the heat exchanger; clamp and remove water lines. Remove oxygen line from gas
inlet port.
2. Clamp venous line at reservoir. Turn off blood pump and clamp arterial outlet.
3. Remove any monitoring/sample line from oxygenator.
4. Double clamp recirculation line, oxygenator inlet and outlet tubing, and cut tubing between clamps, leaving
adequate lengths for reconnection.
5. To remove oxygenator from holder, first release the heat exchanger support base by pulling the release pin
on the base, and slide the base down the mast.
6. To remove the oxygenator from the CVR, turn the oxygenator approximately 1/8 of a turn in a counter-
clockwise direction.
7. Attach the replacement oxygenator to the CVR by inserting the positioning pegs located on the gas cap into
the coupling slots on the CVR, and turn clockwise until the oxygenator locks into position.
8. Replace the heat exchanger support base onto the holder by sliding the base back up the mast until the
heat exchanger rests securely in the horizontal channel of the base. Secure the support base by inserting
the release pin of the support base into the holder mast.
9. Reconnect the recirculation, oxygenator inlet, and outlet tubing. Band all connections.
10. Connect water lines, unclamp, turn on water source, and inspect for water leaks.

13. With sufficient volume in the CVR, turn on blood pump and slowly fill the oxygenator.
14. Increase blood flow to 5 L/min through recirculation line.
15. Stop pump and clamp recirculation line.
16. Ensure that the entire system is free of leaks and gas bubbles.
17. Remove all clamps from venous and arterial lines and reinitiate bypass.
1. Follow previously listed instructions for use for setup with the following modifications as listed here in
steps 2 through 3.
2. Attach a calibrated means of monitoring negative pressures to either the venous line or the venous
reservoir.
3. If a 1.0 cm (3/8 in) venous line is to be used, first attach a venous port reducing adapter to the 1.3 cm
(1/2 in) venous inlet of the Affinity NT CVR reservoir. Connect the venous line to this reducing adaptor.
4. Follow previously listed instructions for use for priming with the following additions as listed here in steps 5
through 10.
5. Replace the vented white luer caps on the manifold sampling line ports with nonvented luer caps. Place
plugs (“dead-enders”) on all unused 0.6 cm (1/4 in) and 1.0 cm (3/8 in) ports.
6. Prime, debubble, and complete preparation of the circuit as per standard procedure. Prime and zero the
venous line pressure monitor.
7. Before instituting bypass, prepare the vacuum portion of the setup. Attach a vented vacuum line to the
0.6 cm (1/4 in) reservoir vent port and to the vacuum regulator with vapor trap.
Warning: Clamp the quick prime line before application of vacuum.
Warning: Do not allow the vapor trap to become completely filled during use. This may allow fluid to enter
the vacuum controller, or may prevent the reservoir from being vented to atmosphere when no vacuum is
applied.
8. To vent the vacuum line, ensure that the “Y” connector with side tubing is in a location that is easily
accessible and leave open to atmosphere. This may be clamped and unclamped regularly throughout the
procedure to apply or stop vacuum (Figure 11).
Warning: Do not exceed vacuum levels greater than −60 mmHg as measured in the venous line.
9. Check to see if the vacuum seals are secure before initiating bypass.
Note: Vacuum will result in increased flow through the sampling manifold line during use.
10. Use vacuum assistance as needed to institute and maintain cardiopulmonary bypass.

Warning: Do not apply vacuum to the venous reservoir when there is no forward blood flow through the
oxygenator. This applies to both arterial centrifugal and roller pumps (roller may not be occlusive in all
positions). This will prevent air from being pulled across the membrane into the blood path by the reservoir
vacuum.
Warning: Do not allow the venous reservoir to become pressurized as this could obstruct venous drainage,
force air retrograde into the patient, or cause air to enter the blood path of the oxygenator.
11. Additional information
The following additional information and data are available upon request: sterilization method, materials of the
blood pathway, plasma leakage across the semipermeable membrane, blood and gas pathway pressure drop at
the maximum blood and gas flow rates, blood cell damage, particle release, air-handling capability, antifoam
characteristics, breakthrough volume, filtration efficiency, relevant tolerances for data in the Instructions for Use,
protocol summaries, and test methods used to determine coverage and leaching.
12. Important notice — limited warranty (US customers only)
A. This LIMITED WARRANTY provides the following assurances to the patient who receives an Affinity NT
integrated Trillium CVR/oxygenator with Trillium biosurface, hereafter referred to as “The Product”.
(1) Should the Product fail to function within normal tolerances due to a defect in materials or workmanship
prior to its Use-by date, Medtronic will, at its option: (a) issue a credit to the purchaser equal to the
Purchase price, as defined in Subsection A(2), against the purchase of the replacement Product or (b)
provide a functionally comparable replacement Product at no charge.
(2) As used herein, Purchase Price shall mean the lesser of the net invoiced price of the original, or current
functionally comparable, or replacement Product.
B. To qualify for the LIMITED WARRANTY, these conditions must be met:
(1) The Product must be used prior to its Use-by date.
(2) The unused portion of the Product must be returned to Medtronic and shall be the property of
Medtronic.
(3) The Product must not have been altered or subjected to misuse, abuse, or accident.
(4) The Product must be used in accordance with the labeling and instructions for use provided with the
Product.
C. This LIMITED WARRANTY is limited to its express terms. In particular:
(1) Except as expressly provided by this LIMITED WARRANTY, Medtronic is not responsible for any direct,
incidental, or consequential damages based on any defect, failure, or malfunction of the product,
whether the claim is based on warranty, contract, tort, or otherwise.
(2) THIS LIMITED WARRANTY IS MADE ONLY TO THE PATIENT IN WHOM THE PRODUCT WAS

INCLUDING BUT NOT LIMITED TO, ANY IMPLIED WARRANTY OF MERCHANTABILITY, OR
FITNESS FOR A PARTICULAR PURPOSE, WHETHER ARISING FROM STATUTE, COMMON LAW,
CUSTOMER OR OTHERWISE. NO EXPRESS OR IMPLIED WARRANTY TO THE PATIENT SHALL
EXTEND BEYOND THE PERIOD SPECIFIED IN A(1) ABOVE. THIS LIMITED WARRANTY SHALL BE
THE EXCLUSIVE REMEDY AVAILABLE TO ANY PERSON.
(3) The exclusions and limitation set out above are not intended to, and should not be construed so as to
contravene mandatory provisions of applicable law. If any part or term of this LIMITED WARRANTY is
held to be illegal, unenforceable or in conflict with applicable law by a court of competent jurisdiction, the
validity of the remaining portions of the LIMITED WARRANTY shall not be affected, and all rights and
obligations shall be construed and enforced as if this LIMITED WARRANTY did not contain the
particular part or term held to be invalid. This LIMITED WARRANTY gives the patient specific legal
rights. The patient may also have other rights which vary from state to state.
(4) No person has any authority to bind Medtronic to any representation, condition, or warranty except this
LIMITED WARRANTY.
Note: This LIMITED WARRANTY is provided by Medtronic, 710 Medtronic Parkway, Minneapolis, MN
55432. It applies only in the United States. Areas outside the United States should contact their local
Medtronic representative for exact terms of the LIMITED WARRANTY.
13. Important notice — limited warranty (for countries outside the US)
A. This LIMITED WARRANTY provides assurance for the purchaser who receives an Affinity NT integrated
Trillium CVR/oxygenator with Trillium biosurface, hereafter referred to as the “Product”, that, should the
Product fail to function to specification, Medtronic will issue a credit, equal to the original Product purchase
price (but not to exceed the value of the replacement Product) against the purchase of any Medtronic
replacement Product used for that patient.
The Warnings contained in the product labeling are considered an integral part of this LIMITEDWARRANTY. Contact your local Medtronic representative to obtain information on how to process a claim
under this LIMITED WARRANTY.

B. To qualify for the LIMITED WARRANTY, these conditions must be met:
(1) The Product must be used prior to its Use-by date.
(2) The Product must be returned to Medtronic within 60 days after use and shall be the property of
Medtronic.
(3) The Product may not have been used for any other patient.
C. This LIMITED WARRANTY is limited to its express terms. In particular:
(1) In no event shall any replacement credit be granted where there is evidence of improper handling,
improper implantation or material alteration of the replaced Product.
(2) Medtronic is not responsible for any incidental or consequential damages based on any use, defect or
failure of the Product, whether the claim is based on warranty, contract, tort, or otherwise.

3. Показания за употреба
Affinity NT, интегрираният CVR Trillium/оксигенатор с биоповърхност Trillium, е предназначен за употреба
в екстракорпорална перфузионна верига, за да събира венозната и всмуканата кръв за кардиотомия, да
охлажда или затопля кръвта и да оксигенира и отстранява въглеродния диоксид от кръвта по време на
рутинни кардиопулмонални байпас процедури (CPB) с продължителност максимум 6 часа.
4. Противопоказания

5. Предупреждения
Прочетете внимателно всички предупреждения, предпазни мерки и инструкции за употреба преди
работа. Ако не сте прочели и не следвате всички инструкции или не съблюдавате всички
предупреждения, това може да причини сериозно увреждане или смърт на пациента.
■
Пътят на течността е стерилен и непирогенен. Огледайте всяка опаковка и устройство внимателно
преди употреба. Не използвайте устройството, ако опаковката е отворена или повредена, или ако
устройството е повредено, или предпазните капачки не са на мястото си.
■
Ако се наблюдават въздушни мехурчета и/или течове по време на подготовката и/или операцията,
тези състояния могат да причинят въздушна емболия на пациента и/или загуба на течност.
Екстракорпоралната верига трябва да бъде следена непрекъснато. Не използвайте устройството, ако
се наблюдават тези състояния.
■
Отстранете всички газови емболи от екстракорпоралната верига преди започване на байпаса.
Газовите емболи са опасни за пациента.
■
При всяка екстракорпорална верига трябва да се обмисли използването на филтър преди байпаса и
за артериалната линия.
■
Трябва да се обмисли проследяване на налягането във веригата преди оксигенатора.
■
През цялото време налягането на кръвната фаза трябва да бъде по-високо от налягането на
газовата фаза в оксигенатора.
■
Не надхвърляйте налягане на кръвната фаза от 750 mmHg.
■
Не надхвърляйте налягане на водната фаза в топлообменника от 206,8 kPa (30 psi).
■
Температурата на водата в топлообменника не трябва да надхвърля 42°C (107°F).
■
Следвайте протокола за кардиопулмонален байпас на медицинското заведение, когато използвате
дезинфектанти в нагревателя/охладителя по време на байпас. Интегритетът на водния път е
потвърден с водороден пероксид (330 ppm). Свържете се с Medtronic за информация относно
използването на допълнителни дезинфектанти.
■

■

време на операция.
■
Само лица, които са подробно обучени в извършването на процедури с кардиопулмонален байпас,
