use of this Optometric Eye chart will help you
determine the clarity of person distance vision.
This can be useful to screen children for nearsightedness, which causes blurry distance vision.
Only a trained eye care professional
can properly diagnose vision disorders,
home use of an Optometric chart is not
a replacement for professional vision testing. While
invaluable for determining near-sightedness, the
optometric chart does not detect any of other
vision disorders.
The optometric chart can be cleaned using
a mild cleaning solution and a soft cotton cloth
or swab. Do not use any paint thinner or
alcohol. It may damage the quality of Chart Sheet
and fade Inks.
Before Checking Visual Acuity, please ensure
person is standing/seating on distance that
mentioned on the Chart. 10Feet or 20Feet.
(3meter, 4meter or 6meter)
Equipment Required
• Multi-letter Optometric Eye chart (Any type:
Snellen, OPTO Type, SLOAN, LOGMAR,
SYMBOLS, NUMBERS)
• E or C Snellen chart or a chart with
illustrations for patients who cannot read
or speak.
• Plain Occluder (not essential)
• Ruler or measuring tape
• Patient's documentation.
Important Note:
• Ensure good natural light or illumination
on the chart.
• Wash and dry the occluder and pinhole.
If no plain occluder is available, ask the patient to
wash his/her hands as they will use a hand to
cover one eye at a time.
• Test each eye separately - the ‘bad’ eye rst.
• Ask the patient to wear any current distance
spectacles, to cover one eye with his/her hand (or
with a plain occluder), and to start reading
from the top of the chart.
How to USE:
a) Secure the Snellen chart to a at surface in a
well-lit room. The chart should be at a comfortable
height, which may change depending on
the individual's height.
b) Measure twenty feet from the chart and mark
a spot facing the chart directly. (The test results
will only be accurate if it is taken from this
distance.)
c) Stand at the twenty-foot line and cover your
left eye, so you can only see out of your right eye.
Starting from the top, read each row from left to
right for as far down as you can still make out
the letters. Note the last line on which you could
correctly identify every letter. Have an assistant
verify that you are reading the letters correctly.
d) Repeat the test with your left eye, covering your
right eye this time. Note the last row you could
read with complete accuracy. The row for
each eye will not necessarily be the same.
e) Take note of the fraction shown to the left of
each row: It identies how your vision rates. For
example, if you could read to the fth line,
you have 20/40 vision; if you could read to the
seventh line, you have 20/25 vision.
f) Look at your results to determine the quality
of your eyesight. For example, someone with
20/60 vision can read at 20 feet away what a
person with normal vision could read at 60 feet
away. In the United States, a score of 20/40 or
better is required to drive a car without
corrective lenses, vision of 20/200 or worse
qualies an individual as legally blind. Explain
the procedure to the patient.
g) In the patient's documentation, record the VA
for each eye, stating whether it is with or without
correction (spectacles)
Example, if using the E or C chart:
h) Point to each letter on each line and ask
the patient to point in the direction toward which
the open end of the letter is facing
i) Follow the same procedure and recording
methods as above.
