INSTRUCTIONS FOR USE
Before curettage, a local anæsthetic is generally required, with appropriate disinfection of the skin. In cases of very small, superficial lesions, it is possible to avoid the anaesthetic.
1. The most commonly-used method is the ”fountain-pen technique“. To do this, the curette is held between the thumb, index and middle finger, while the skin should be stretched with the fingers of the other hand.
2. This method of holding enables precise guiding of the instrument, so that the piece of tissue can be removed in one well-targeted incision.
3. A further commonly used method is the ”potato-peeler technique“, held the handle of the curette in the distal inter­digital fold of the index finger, and supported by the other fingers of the curetting hand. The thumbs serve to provide a stable base. This technique makes it possible to guide the instrument, applying greater pressure, but accuracy is reduced somewhat in so doing. In view of the sharpness of the curette, it should preferably be deployed only in cases of hard skin.

The biopsy occupies a central place in medical diagnosis. Despite the development of diagnostics procedures, histopathological
tissue assessment continues to form the ”Gold Standard“. Given
its simplicity of handling, as well as the presentation of a skin test that is neatly defined in terms of its length and diameter, the punch biopsy is currently the most suitable instrument to take skin biopsies. The punch biopsy gives a representative cross-section through the entire cutis, as it reaches sufficiently deep and includes a sufficiently wide excision of the cutis. Usually a 4mm punch will suffice.

INSTRUCTIONS FOR USE
The biopsy itself is technically straightforward and can be performed very rapidly. It is carried out under a local anaesthetic.
1. The skin is stretched out to the relaxed skin tension lines
2. The punch is held between the thumb and index finger,
placed upon the upper surface the skin and pressed down clockwise in a “screwing” motion at moderate pressure. It is important that penetration into the fatty tissue is sufficiently deep to obtain a suitable sample. The right length of the cut is 7mm. A longer biopsy can be obtained through intra­operative pressure. The operator’s technique will determine the depth of the tissue taken.
3. The biopsy should be carefully caught from the punch using
a small anatomical set of tweezers, avoiding any crushing. It is then separated away flat with the scissors or scalpel at the base and placed over a suitable container.
4. Due to tensioning of the skin, an oval-shaped blemish is
produced, which can easily be closed up. Hæmostasis using an instrument is almost never needed. In the case of punches up to 4mm diameter, close the wound with suture. The resulting scars are scarcely visible.
