HL7StandardBackground

TheHealthLevelSevenStandard(HL7)isusedtoexchangedatabetweencomputer systems.Itdoesnotrequireaspeciccomputeroperatingsystem,programming language,orcommunicationprotocolforitsimplementation.
ThegoaloftheHL7Standardistostandardizemessagecontentandusage,while allowinguser-specicvariationswithinthestandard.Toaccomplishthis,theHL7 Standardspeciesencodingrulesusedtocreatethemessageformat.Basedonthese rules,themessagesgenerallyconsistofdataeldsanddatasegments.
Amessageiscomprisedofmultiplesegments.Whilesomeofthesegmentsare requiredtocreateamessage,othersareoptional.EachsegmentwithintheHL7 messageisseparatedbyspecialsegmentseparatorcharacters.

Eachsegmentcontainsanumberofdataelements.Thedataelementsmaybeof varyinglengths.Likethesegments,elementsareseparatedfromeachotherbyspecial characters.Anumberofdataelementsandtheirseparatorsarelogicallygrouped togethertocreatedatasegmentssuchasthemessageheadersegmentorthepatient identicationsegment.Withtheexceptionofthesegmentseparatorcharacters,the datacontainedinHL7messagesconsistsofdisplayableASCIIcharacters.
Eachsegmentbeginswithathreecharactervalue,forexample“MSH”forthe messageheadersegment.Thesethreecharactersuniquelyidentifythesegment withinagivenmessage.Segmentsareidentiedaseitherrequiredoroptional,and somemayberepeated.Similartodataelds,datasegmentsareseparatedfromeach otherbysegmentseparatorcharacters.
BasedontheHL7encodingrules,eachmessagewithintheHL7protocolhasaknown structure.Thesegmentsanddataeldsthatcompriseagivenmessagearealways thesame,plusorminusthedenedoptionalsegmentsanddataelds.Asaresult,an individualdataeldcanbefoundwithinamessagesimplybyknowingitscongured positioninasegment.
HL7messagesarepassedbetweencomputersystemsaspartsofvalidtransactions. Forexample,admittingapatientontheHospitalInformationSystem(HIS),orreceiving acompletedstudyresultontheMUSEsystemwouldcauseanHL7messagetobe generatedandsent.Afteramessageissent,thereceivingsystemprocessesthe message.Whenprocessingiscomplete,thereceivingsystemcanprocessthenext message,oritcanoptionallygenerateanapplication-levelacknowledgmentthatis returnedtothesendingsystem.

Introduction

TheMUSEHL7Interfacedoesnotsupportinboundbatchmessages.

Low-levelCommunications

Thespecicinstallationdetermineswhichlow-levelcommunicationprotocolis used.TheHL7interfacestandardisdesignedtoaccommodateawidevarietyof communicationmethodologies,frommessage-basedcommunicationstoletransfer schemes.
Implementationofthelow-levelcommunicationprotocoldoesnotdirectly affecttheHL7interfacemessagecontentandfunctionality.Notalllow-level communicationprotocolssupporthigh-levelacknowledgments.Therefore,if high-levelacknowledgmentsaredesired,alow-levelprotocolthatsupportsthem mustbeselected.
TheHL7interfaceassumesthatthelow-levelcommunicationprotocolensuresthat thedataarriveserror-free.Asaresult,nodataintegritycheckingisdoneatthe applicationlevel.TheMUSEHL7interfaceallowsonlyTCP/IPsocketcommunication forreceivinginboundHL7transactions.

TheHL7ADTtransactionstransmitpatientdemographicandpatientvisitinformation fromthehospital’sADTsystemtotheMUSEsystem.Neworupdatedpatient informationisenteredintothehospital’sADTsystemandsenttotheMUSEsystemas anunsolicitedtransaction.MUSEsystemuserscanthenviewthepatientinformation receivedfromtheHIS.TheMUSEsystemwillnotacceptbatchADTmessages.
Theorderentrytransactionstransfertheorderinformationforarequestedstudyfrom ahospital’sorderentrysystemtotheMUSEsystem.Allordersoriginateonthehost systemandaresenttotheMUSEsystemasunsolicitedtransactions.

TransactionsFromtheMUSEHL7Interface

TheMUSEsystemmanagesandstorespatientstudiessuchasrestingECGs,Holters, andexercisestressstudies.Initially,thestudiesareperformedonancillaryMUSE devicesandthestudyresultsarethenacquiredintotheMUSEsystemforprocessing. Atacquisition,thestudiesaregenerallyunconrmedwithastatusofpreliminary whichmeansthattheresultshavenotbeeninterpretedorreadbyaphysician.When astudyisinterpretedorreadbyaphysicianandconrmed,thestatusofthereport updatestonal.Thestoredresultscanberetrievedand/oreditedasnecessary.Tests thatarere-editedandsavedareincorrected/revisedstatus.
TheMUSEReportDistributionsetuputilityisusedtocongurethegenerationofresult andnancialmessagestotheMUSEinterfaceengineandthentotheHIS.Aswithother reportdistributioncongurations,eachMUSEstudytypehasitsownconguration. Thereportdistributioncongurableparameterswithineachstudytypeare:
•Patient/studylocationparameter—ThesetupofthelocationparameterforHL7 messagingconformstothesamerulesasotherreportsordevices.Refertothe MUSECardiologyInformationSystemOperator’sManualfordetailedconguration.
•Triggersforgenerationofmessage—Theavailabletriggersforgenerationofan HL7messagearethesameasthoseforreportstodevices.SettingupanHL7 resultintheunconrmedsectionoftheReportDistributionutilitygeneratesan HL7messageonacquisitionofthestudyfromtheancillarydevicetotheMUSE server.EntriesintheDemographicsCompletesectionareexecutedwhenthe studyismarkedas"DemographicsComplete.”Entriesintheconrmedsection aregeneratedwhenthestudyisconrmedandwhenthestudyisrevisedand re-conrmed.Iftheformatis“HISbilling,”theHL7messageisgeneratedonlyonce foreachtrigger ,eveniftheactiontotriggeranothermessageisexecuted.Special userprivilegesarerequiredtogenerateadditionalbillingmessages.
