Thank you for purchasing the Form Flip Table. This
page lists all the contents included in the box. Please
take time to identify the hardware as well as the
individual components of this product. As you unpack
and prepare for assembly, place the contents on a
carpeted or padded area to protect them from damage.

Note: 2 people are required to safely assemble this product.

NOTE: Do not over
tighten the bolts

1 With the assistance of another adult, place tabletop onto a clean, padded
surface. Attach 4 legs to tabletop with 2 flat washers , 2 spring washers and 2

bolts per leg as shown. Secure, but do not over tighten, the bolts using an allen
wrench. Position unit upright.

Assembly is now complete.

INSTRUCTIONS FOR
TABLE EXTENSION

1 Release safety locks on
underside of tabletop. Carefully
slide tabletop as shown.

UNLOCK

2 Carefully unfold right side of
tabletop as shown. Once
tabletop is flat, make sure
safety locks are secured to
prevent table from moving
during use.

Assembly is now complete.

CLEANING AND CARE

NOTE: To prevent tipping,
do not sit or put weight on
table end

Clean surfaces with a dry or damp soft cloth. Do not use abrasive cleaners.
Do not use furniture wax or polish as the wood is finished with a protective
clear lacquer. It is recommended to use coasters to protect surface from heat
and water.
