Rebate Redemption Instructions:
1. Mail the completed rebate certificate including model and serial numbers to the address below for eligible models purchased between June 1, 2020
and December 31, 2020. Purchase date is determined by the sales invoice date. Keep copies for your records.
2. Include a legible copy of your sales invoice showing model number, purchase date, and purchase price for each item.
3. Envelope must be postmarked by January 31, 2021 and last date of re-submission is February 28, 2021.
4. If you have not taken delivery of your purchases you must submit a completed rebate form and sales invoice, without the seri al numbers, by
January 31, 2021. You must then call (800) 966-8300 by February 28, 2021 with your serial numbers or else the rebate will be void.
5. Allow 4 - 8 weeks from the time we receive all required information for delivery of your check.
